Change Order No. 01 Western Avenue Corridor Improvements Project No. 115-097 The following sections of the Contract Documents are revised as a result of the above changes: BIDDING AND CONTRACT REQUIREMENTS: Bid/Proposal Form: NO CHANGE SPECIFICATIONS: The following special provisions have been amended or added to the Contract Documents and are issued as part of this Change Order: XIII. PROSECUTION AND PROGRESS B. Additions 3. The project will have intermediate completion dates as listed in the table below for substantial completion: Phase I & 11: Kaley Street and Meade Street Sewer and Water Crossings — June 30, 2017 Phase III: South Side Improvements — August 31, 2017 Phase IV: North Side Improvements — October 10, 2017 Phase V: Fixtures, Trees, Fencing, and Landscaping — October 13, 2017 a. The project will have an intermediate substantial completion date of June 30, 2017 for storm sewer to be constructed from the intersection of Western Avenue and Kaley Street to its northern connection on Kaley Street and for water crossings at Meade Street and at Kaley Street, A minimum of one travel lane and one parking lane along Western Avenue shall be open to traffic during the lane closure. b. The project will have an intermediate substantial completion date of August 31, 2017 for construction of curb, sidewalk, roadway, lighting, pavers, and all project elements complete and in -place on the southern side of the roadway centerline except for pavement markings, benches, bike racks, landscaping, and trees. A minimum of one travel lane and one parking lane along Western Avenue shall be open to traffic during the lane closure period. Temporary parking lane closures will be permitted for water service connections. Phase III of the project can occur simultaneously with Phase II, provided that the contractor maintains a minimum of one travel lane along Western Avenue and submits a revision to the Maintenance of Traffic Plan that is approved by the City prior to construction. c. The project will have an intermediate substantial completion date of October 10, 2017 for construction of curb, sidewalk, roadway, lighting, pavers, pavement markings, and all project elements complete and in -place on the north side of the roadway centerline except for benches, bike racks, landscaping, and trees. A minimum of one travel lane and one parking lane along Western Avenue shall be open to traffic during the lane closure period. d. The project will have an intermediate substantial completion date of October 13, 2017 for installation of benches, bike racks, landscaping, fencing, and trees. A minimum of two travel lanes on Western Avenue shall be open to traffic during the lane closure period. XXX. CONCRETE PAVEMENT REMOVAL SHEET 2 OF 3
