Notes: <br />Change Order No. 12 addresses "Request for Proposal No. 3" which the City requested pricing <br />for after Arcadis U.S. completed the design effort previously approved by the Board of Public <br />Works. The cost for this proposal includes value engineering costs from by Thieneman <br />Construction in conjunction with the proposal pricing supplied for Change Order No. 12. This <br />proposal was requested as an additional scope of work. The Aeration Gallery Deck had <br />originally been omitted from the project as a cost saving measure during design. Low costs <br />during bidding provided an opportunity to address safety and access deficiencies while the <br />General Contractor is mobilized and performing work at the Aeration Gallery. The resultant <br />effort results in an increase in costs and will providing better access to the Aeration Gallery, <br />Aeration Tanks 1-4 and Final Clarifiers 1-5. <br />The funding sources for the construction of this project are the 2012 Sewer Bond (661-0630- <br />415-42-06) and Wastewater Equipment Purchases / Treatment and Disposal (642-0630-415-43- <br />66). This approval is subject to a 3rd Quarter Appropriation that will move bond funds from <br />Sewer to Wastewater Treatment Plant for the purposes of paying a portion of Change Order No. <br />12. The expected split is $78,742.04 from the 2012 Sewer Bond and $404,668.96 from the <br />Wastewater Equipment Purchases / Treatment and Disposal Account.
