South Bend, IN 46628 RE: ADA Restroom <br />Attention: Jessica Alstott <br />April 20, 2017 <br />Gibson -Lewis, L.L.C. is pleased to offer the following proposal for <br />your review. <br />Scope of Work: <br />1. Remove existing flooring and ceiling in large storage room in preparation for creating <br />(2) separate spaces. <br />2. Construct a new wall to create two separate rooms. <br />3. Furnish and install (1) new door, frame, and hardware to match existing. <br />4. Remove the existing water heater and save for relocation. <br />5. Disconnect and remove all plumbing piping back to a point suitable for reconnection. <br />6. Remove the existing return grille and cap the ductwork above the ceiling. <br />7. Remove the supply grille and water heater B-vent to a point suitable for <br />reconnection. <br />8. Perform the necessary layout for the required floor cutting in the space. <br />9. Furnish and install a complete Schedule 40 PVC sanitary waste and vent system for <br />the new restroom and utility room and connect to the existing piping. <br />10. We cannot tell what the existing underground is in this room that is feeding the floor <br />drain. If we remove the floor drain and find a 3" line, we will install a normal pressure <br />assist tank toilet. If we remove the floor drain and find a 2" line, we will install a <br />pumped toilet system. <br />11. The new vent piping for the fixtures will connect to the existing vent piping above the <br />existing men's locker room. <br />12. The new sanitary will pick up the new water closet and lavatory in the restroom as <br />well as a new floor drain in the utility room. <br />13. Furnish and install a complete Type "L" copper domestic water system for the new <br />restroom and connect to the existing piping. <br />14. Furnish and install a complete Schedule 40 carbon steel natural gas system for the <br />relocated water heater and connect to the existing piping in the space. <br />15. Furnish and install the following plumbing fixtures and equipment: <br />• 1— Water Closet: Kohler pressure assist tank toilet or Zoeller pump toilet <br />system with ADA height bowl, seat and all applicable trim. <br />• 1— Lavatory: Kohler wall hung lavatory with Kohler chrome single handle <br />faucet, ADA trap guard, point -of -use mixing valve (code required) and all <br />applicable trim. <br />1— Water Heater (Relocated): Relocate the existing water heater, reconnect <br />to required piping and provide relief valve discharge to floor drain. <br />A NATIONAL CONSTRUCTION ENTERPRISES COMPANY <br />
