REGULAR MEETING APRIL 11, 2017 101 WILLIS CONSTRUCTION 54300 Rose Road South Bend, IN 46628 Quotation was submitted by Joshua Willis QUOTATION: $10,400.00 JW CONCRETE & EXCAVATING 1617 Td Street Osceola, IN 46561 Quotation was submitted by Rodney Rouch QUOTATION: $12,755.00 Upon a motion made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the above quotations were referred to Engineering and the Fire Department for review and recommendation_ After reviewing those quotations, Mr. Kevin Fink, Engineering, informed the Board that TX Concrete did not fill in unit prices, just totals for each line item. Mr. Fink stated he calculated the unit prices based on quantity and totals and Mr. Jose Quijado, TX Concrete, confirmed over the phone that his calculations were correct. Mr. Fink stated Willis construction made a multiplication error that, when corrected, results in a $69 increase to the final total. He noted he confirmed this with Willis Construction over the phone also. Mr. Fink stated the quote tabulation reflects the corrected final amount, and the increase does not impact the quote order results. Therefore, Mr_ Fink recommended that the Board award the contract to the lowest responsive and responsible bidder, TX Concrete, in the amount of $8,545.13. Upon a motion by Ms. Maradik, seconded by Ms. Fritzberg and carried, the recommendation was approved and the quotation was awarded as outlined above. OPENING OF QUOTATIONS — NORFOLK SOUTHERN AND CANADIAN NATIONAL RAILROAD CROSSINGS AND QUIET ZONES MATERIALS— PROJECT NO. 116-110 LOIT Mr. Gilot advised that this was the date set for the receiving and opening of sealed Quotations for the above referenced project. The following Quotations were opened and read: IMPACT RECOVERY SYSTEMS, INC. 4955 Stout Drive San Antonio, TX 78219 Quotation was submitted by Carol Pollok Contractor's Non -Collusion Affidavit, Non -Debarment Affidavit, Certification Regarding Investment with Iran, Employment Eligibility Verification, Non -Discrimination Commitment for Contractors and Certification of Use of United States Steel Products or Foundry Products was submitted QUOTATION: $42,298.63 MGI TRAFFIC CONTROL PRODUCTS PO Box 421 Fairland, IN 46126 Quotation was submitted by Mike Green Contractor's Non -Collusion Affidavit, Non -Debarment Affidavit, Certification Regarding Investment with Iran, Employment Eligibility Verification, Non -Discrimination Commitment for Contractors and Certification of Use of United States Steel Products or Foundry Products was submitted QUOTATION: $45,966.40 Upon a motion made by Ms. Maradik, seconded by Ms. Fritzberg and carried, the above Quotations were referred to Engineering for review and recommendation.
