TO groups Kim] <br />staff trained, parents participating, and youth participating) In these prograws. <br />The following outlines the proposed evaluation format. The various forms wi l l <br />be developed by Bureau staff in conjunction with our local Mental Health Center <br />consultant. <br />1. Bureau staff being trained - quarterly evaluation of the experience. It <br />would request subjective information so as to gent a sense of hog•! staff Feel <br />ski 1 is are heinq improved. It will also ask for how staff is uti 1 izing new <br />knowledge in terms of individual, group, and rnwiy sessions. Thure will W <br />a second separate tool to be Fi l led out after each of the two 2 -day workshoo. <br />This will be to evaluate the experience for Youth Service Bureau administra- <br />tive purposes and to provide feedback to facilitators. <br />2. For .parents - Immediately after parent group concludes, Lhuy will be ask to <br />fill out a questionnaire that addresses itscI F to the fuelings level. Such <br />questions as "Does this experience make you feel more hopeful ", etc. at this <br />time. Approximatuly one month after the parent group, a Youth Service Bureau <br />intern will go to individuals homes to fill out a questionnaire that would <br />ask specific questions about the value of the expeA ence, what kind of sug- <br />gestions and teachings were utilized in the hone setting, etc. <br />3. As part of our follow-up care, we contact parents and the client individually <br />after two months. This is done to assess the value of our intervention and <br />to provide more services where. necessary. <br />4. As a way of further assessing our value, l:e wi l l Keep a statistical report <br />of the number of youth seen at the shelter, how many went 'come, how many <br />returned to the shelter a second or third time, and how many stayed at hom:_ <br />until age of majority. This will he an on -going progra m requi rcrunt. <br />
