1/3/72 <br />16 <br />LETTER -CATHOLIC CHARITIES DAY CARE CENTER <br />The following letter was filed with the Board of Works: <br />State of Indiana <br />State Board of Health <br />Catholic Charities <br />c/o Crumlish-Sporieder & Associates <br />100 Center Street <br />Mishawaka, Indiana 46544 <br />Gentlemen: <br />Indianapolis <br />Indiana State Board of Health <br />1330 West Michigan Street <br />Indianapolis, Indiana 46206 <br />December 28, 1971 <br />Re: Approval of Plans and Specifications for <br />Day Care Center for Catholic Charities <br />2208 Linden, South Bend, St. Joseph County <br />The plans and specifications for the proposed day care center for <br />914 Catholic Charities, to accommodate 40 children, have been reviewed and are <br />hereby approved this 28th day of December, 1971. <br />Lf� <br />(.� This project includes the construction of a 2-inch water supply connection <br />U and a 6-inch sanitary sewer connection to local facilities. <br />U <br />This approval is given with the following conditions: <br />1. That all required local permits be obtained before construction <br />is begun on this project. <br />2. That plans and specifications for any changes, alterations <br />additions to this child care facility as herewith approved <br />be submitted to and approved by the State Board of Health <br />prior to such construction. <br />3. That the Children's Division, Department of Public Welfare <br />be notified in time to schedule final inspections before any <br />children are admitted to the facility. <br />These plans and specifications were prepared and certified by Brian <br />Crumlish, A.I.A.,'and Donald E. Sporleder, A.I.A., Mishawaka and submitted <br />on November 17 and December 14, 1971. <br />This approval shall be void if construction is not begun before January <br />1, 1973. <br />Sincerely, <br />(s) A.C. Offutt, M.D. <br />State Health Commissioner <br />Indiana State Board of Health <br />Upon motion made, seconded and carried the above letter was referred to the <br />Building Department. <br />EXCAVATION BONDS <br />Upon motion made, seconded and unanimously carried, the Board approved <br />the following: <br />Excavation bond for Indiana and Michigan Electric Co. to 1/1/73 <br />Continuation certificates for: <br />Delbert M. Hull to 1/1/73 <br />Midland Engineering Co. to 1/1/73 <br />0. J. Shoemaker to l/l/73 <br />
