REGULAR MEETING <br />NOVEMBER 4, 1974 <br />RECOMMENDATION TO RELEASE BOND <br />Ray S. Andrysiak, Bureau of Engineering, recommended to the Board <br />that the request to release the bond of Van Overberghe Builders <br />had been reviewed and the bond could be released. Upon motion <br />made, seconded and cajried, the Contractor's Bond for Van Overberghe <br />Builders, Inc. was released. <br />EXTRA WORK AGREEMENT APPROVED - FIRE STATION NO. 4 <br />The Board received an Extra Work Agreement in the amount of $549 <br />from Muia Construction Company for the construction of Fire Station <br />No. 4. Ed Bauer, Building Department, was present and explained <br />that this was for the installation, at the request of Ralph Wadzinski, <br />of one -inch conduit for the installation of a flasher system at <br />the station and for face brick. Upon motion made, seconded and <br />carried, the Board approved the Extra Work Agreement. <br />STREET LIGHT OUTAGE REPORT <br />The Street Light Outage Report for the period of October 22 <br />through 27 was received and ordered filed. <br />APPROVAL OF COMPLETION AFFIDAVIT —MAYFLOWER ADDITION SEWERS <br />A Project Completion Affidavit for Sanitary and Storm Sewers <br />in Mayflower 5th Addition was submitted to the Board by the <br />Colpaert Realty Corporation. A Maintenance Bond was attached. <br />Upon motion made, seconded and carried, the Board approved the <br />completion affidavit for sewers only.and the Maintenance Bond. <br />RECOMMENDATION FROM LEGAL DEPARTMENT RE: PROJECT COMPLETION <br />AFFIDAVIT FOR BURNETT INDUSTRIAL PARK <br />The Board received a recommendation from Kathleen Cekanski, Deputy <br />City Attorney, that she had reviewed the Project Completion Affidavit <br />for the Burnett Industrial Park. Miss Cekanski informed the Board <br />that, since the Completion Affidavit was signed by Robert Kertz <br />of the Sollitt Construction Company, who has power of attorney to <br />sign for Burnett, she would recommend that the Board request that <br />Mr. Kertz furnish them with a power -of -attorney in his name to be <br />attached to the Completion Affidavit. When that has been received, <br />the Affidavit can be approved. Upon motion made, seconded and carried, <br />the Board accpeted the recommendation and requested that Mr. Kertz <br />be asked to furnish a power -of -attorney in his name to the Board. <br />AMENDMENT NO. 2 - TEAMSTERS AND BUREAU OF STREETS APPROVED <br />The Board received Amendment No. 2 to the Agreement between the <br />Teamsters and the Bureau of Streets, Municipal Services Facility, <br />Traffic and Cemeteries with a recommendation from James A. Roemer, <br />City Attorney, that the amendment could be approved. Upon motion <br />made, seconded and carried, the Board approved the amendment. <br />ri <br />
