a <br />REGULAR MEETING OCTOBER 3, 1983. <br />Mr. Leszczynski further advised that in accordance with <br />the award of this bid, a contract in said amount was. <br />being submitted for Board approval. Upon a motion made <br />by Mr. Leszczynski, seconded by Mr. Hill and carried, the <br />above bid in the amount of $73,607.50 was awarded and the <br />contract approved. Accordingly, the appropriate insurance, <br />Performance Bond and Labor and Materials Payment Bond <br />submitted by The Robert Henry'Corporation were filed. <br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION_ <br />AFFIDAVIT - MONROE PARK PHASE IV PROJECT (CURB AND LANDSCAPING <br />IMPROVEMENTS) - (GEYER CONSTRUCTION AND DEVELOPMENT,COMPANY) <br />Mr. Leszczynski advised that Geyer Construction and Develop- <br />ment Company has submitted Change Order No. 1 (Final) <br />indicating that the contract amount be increased by $11,590..24 <br />for a new contract sum including this Change Order in the <br />amount of $34,050.74. Additionally submitted.was the <br />Project Completion Affidavit indicating this new final cost. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill <br />and carried, Change Order No. 1 (Final) and the Project <br />Completion Affidavit were approved subject to the filing <br />of the appropriate three-year Maintenance Bond. <br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION <br />AFFIDAVIT - CUSHING STREET IMPROVEMENT (NHS) PROJECT - <br />(MC INTYRE AND JONES CONSTRUCTION COMPANY, INC.) <br />Mr. Leszczynski advised that McIntyre and Jones Construction <br />Company, Inc. has submitted Change Order No. 1 (Final) <br />indicating that the contract amount be increased by <br />$12,336.61 for a new contract sum including this Change Order <br />in the amount of $26,254.49. Additionally submitted was <br />the Project Completion Affidavit indicating this new final <br />cost. Upon a motion made by Mr. Leszczynski, seconded by <br />Mr. Kernan and carried, Change Order No. 1 (Final) and the <br />Project Completion Affidavit were approved subject to the <br />filing of the appropriate three-year Maintenance Bond. <br />APPROVE CONSTRUCTION CONTRACT - SEEBIRT PLACE CONCRETE <br />STREET RECONSTRUCTION' <br />Mr. Leszczynski advised that in accordance with the bid <br />awarded on September 26, 1983, to Geyer Construction and <br />Development, 67475 Kenilworth Road, Lakeville, Indiana.in <br />the amount of $24,210.00 for the above referred to project, <br />a contract in said amount was being submitted for Board. <br />approval. Upon a motion made by Mr. Leszczynski, seconded <br />by Mr. Hill and carried, the contract was approved subject <br />to the filing of the appropriate insurance, Performance <br />Bond and Labor and Materials Payment Bond. <br />APPROVE OFFICIAL ACTION - INDIANA DEPARTMENT OF HIGHWAYS - <br />SR 23 (SOUTH BEND AVENUE) AND SR 23 (EDDY STREET) & CORBY <br />In a memorandum to the Board, Mr. Ralph J. Wadzinski, Manager, <br />Bureau of Traffic and Lighting, stated that in conjunction <br />with the Indiana Department of Highways, the Bureau of <br />Traffic and Lighting recommends that the above referred to <br />intersections during the emregency flash sequence, be all - <br />red flash for all purposes. Presented to the Board for <br />approval were three (3) copies of an Official Action from <br />the IDOH recommending this action. Upon a motion made by. <br />Mr. Leszczynski, seconded by Mr. Kernan and carried, the <br />above recommendation was approved and the Official Action <br />forms executed. <br />
