<br />JANUARY 31, 1983
<br />APPROVE CONTRACTOR'S BOND
<br />Mr. Ray S. Andrysiak, P.E., recommended the approval of a
<br />Contractor's Bond for Robert T. Pflugner, effective January
<br />31, 1983. Upon a motion made by Mr. Leszczynski, seconded by
<br />Mr. Hill and carried, the above bond was approved as recommended.
<br />APPROVE TRAFFIC CONTROL DEVICE
<br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill and
<br />carried, the following traffic control device as recommended by
<br />the Bureau of Traffic and Lighting was approved:
<br />REVISE TO HANDICAPPED PARKING PLATE TOW AWAY ZONE (30 days)
<br />100 N. Michigan, west side, lst space S. of Colfax.
<br />APPP.OVE 1ICENSE APPLICATIONS - PUBLIC PARKING FACILITY
<br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill
<br />and carried, the license applications of The National Bank &
<br />Trust Company, agent, to operate a public parking facility at
<br />322 South Lafayette Boulevard, South Bend, Indiana and The
<br />National Bank & Trust Company, Trustee, to operate a public
<br />parking facility at 115 Franklin Place, South Bend, Indiana
<br />were approved and referred to the Deputy Controller's Office
<br />for issuance. It was noted that the required Proofs of
<br />Insurance were attached to the applications.
<br />APPROVE LICENSE APPLICATION - SECONDHAND DEALER
<br />Mr. Leszczynski advised that a renewal application for a
<br />Secondhand Dealers License was received from Frank Romeo
<br />Anastasio, of Miami.Jewelry, 1906 Miami Street, South Bend,
<br />Indiana. He further advised that favorable reports had
<br />been received from the Police Department, Fire Department,
<br />Bureau of Traffic and Lighting and the Department of Code
<br />Enforcement. Upon a motion made by Mr. Leszczynski, seconded
<br />by Mr. Hill and carried, the above application was approved
<br />and referred to the Deputy Controller's Office for issuance.
<br />FILING OF CERTIFICATES OF INSURANCE
<br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Hill
<br />and carried, the following Certificates of Insurance were filed:
<br />Walsh & Kelly, Inc.
<br />LaFree Excavating, Inc.
<br />FILE ENVIRONMENTAL CLEAN-UP OF LOTS
<br />A list containing forty (40) lots ordered cleaned by the
<br />Director of the Department of Code Enforcement was submitted.
<br />It was noted that property owners had been advised to clean
<br />up the lots and had failed to do so. Upon a motion made by
<br />Mr. Leszczynski, seconded by Mr. Hill and carried, the
<br />environmental list was filed.
<br />APPROVE HANDICAPPED PARKING PERMITS
<br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Kernan
<br />and carried, the following ten (10) handicapped parking permits
<br />were approved and referred to the Deputy Controller's Office
<br />for issuance:
