85 <br />REGULAR MEETING APRIL 30, 1984 <br />APPROVE MODIFICATION NO. 01 - ST. JOSEPH COUNTY JOB TRAINING <br />PROGRAM AGREEMENT FORM - REIMBURSEMENT FOR SERVICES ONE HALF-TIME DEPUTY ATTORNEY <br />Mr. Leszczynski presented to the Board Modification No. 01 - <br />JTP Agreement Form for reimbursement services for.one half- <br />time deputy attorney. Modification No. 01 extends the contract <br />end date from March 31, 1984 to July 31, 1984, increases contract <br />funding by $4,850.00 (from $7,250.00 to $12,100.00) to cover <br />legal services provided to the St. Joseph County Job Training <br />Program during the contract extension period, changes the <br />dates on the Program Plan to account for the extension period <br />and modifies the program budget to incorporate the extension <br />period costs. Upon a motion made by Mr. Leszczynski, seconded <br />by Mr. Hill and carried, the above Modification. No. 01 - JTP <br />Agreement Form was approved. <br />APPROVE PROPOSAL FOR ENGINEERING SERVICES - COLE ASSOCIATES, INC. <br />Mr. Leszczynski presented to the Board a Proposal for Engineering <br />Services, with Cole Associates, Inc.,2211 East Jefferson Boulevard, <br />South Bend, Indiana, for engineering services for signalization or <br />reworking the existing traffic signal system at the following <br />intersections: <br />1. Bendix Drive and Voorde Drive <br />2. Calvert Street and Twyckenham Drive <br />3. Logan Street and Mishawaka Avenue <br />4. Hickory Road and Miracle Lane <br />5. Portage Avenue and California Avenue <br />6. Portage Avenue and Forest Avenue <br />It was indicated that this project consists of performing topographic <br />surveys, traffic signal warrant analysis and design engineering <br />services to produce a set of construction documents acceptable <br />by the Indiana Department of Highways to qualify for Federal Aid <br />Funding. It was noted that the total cost of these services shall <br />not exceed $21,000.00. Upon a motion made by Mr. Leszczynski, <br />seconded by Mr. Hill and carried, the above proposal with Cole <br />Associates, Inc. was approved. <br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION <br />AFFIDAVIT - INLET RECONSTRUCTION 1983 PROJECT <br />Mr. Leszczynski advised that Dye Plumbing and Heating, Inc., 3535 <br />Monroe Street, LaPorte, Indiana, has submitted Change Order No. 1 <br />(Final) indicating that the contract amount be increased by <br />$12,690.00 for a new contract sum including this Change Order in <br />the amount of $72,440.00. Additionally submitted was the Project. <br />Completion Affidavit indicating this new final cost. Upon a <br />motion made by Mr. Leszczynski, seconded by Mr. Hill and carried, <br />Change Order No. 1 (Final) and the Project Completion Affidavit <br />were approved and the appropriate three-year Maintenance Bond <br />was filed. <br />APPROVE REQUEST TO ADVERTISE FOR RECEIPT OF BIDS - <br />CONCRETE PAVEMENT REPAIRS 1984 PROJECT <br />In a letter to the Board, Mr. Neil A. Shanahan, Manager, Bureau <br />of Public Construction, requested that the Board advertise for <br />the receipt of bids for the Concrete Pavement Repairs 1984 <br />Project. Upon a motion made by Mr. Leszczynski, seconded by Mr. <br />Hill and carried, the above request was approved. <br />APPROVE REQUEST OF ST. JOSEPH COUNTY DEMOCRATIC CENTRAL <br />COMMITTEE TO USE PUBLIC BUILDINGS FOR VOTING PURPOSES <br />MAY 8. 1984 <br />In a letter to the Board, Mr. C.J. Pajakowski, Chairman, St. <br />Joseph County Democratic Central Committee, P.O. Box 1375, South <br />Bend, Indiana, requested permission to use the following public <br />buildings for voting purposes on Primary Election Day, May 8, 1984: <br />
