74, - <br />REGULAR MEETING <br />APRIL 16, 1984 <br />APPROVE LICENSE APPLICATIONS <br />Mr. Leszczynski advised that the following license applications <br />have been received: <br />MASSAGE TECHNICIAN: <br />MASSAGE ESTABLISHMENT <br />RUBBISH & GARBAGE: <br />PUBLIC PARKING FACILITY: <br />Berthella Stevens <br />69901 Elm Road <br />Bremen, Indiana <br />L'Image Classique, <br />519 East Jefferson <br />South Bend, Indiana <br />By: E., Erika Mays <br />Fred Stepaniak <br />P & R Trash <br />51981 U.S. 31 North <br />South Bend, Indiana <br />Inc. <br />Boulevard <br />President <br />Robert_ Williams <br />Rob' s Parking <br />120 West.Western Avenue <br />South Bend, Indiana <br />Mr. Leszczynski further advised that the Massage Technician applica- <br />tion of Berthella Stevens contained the necessary doctor's certificate <br />and received a favorable recommendation from the Police Department. <br />The Massage Establishment application of L'Image Classique, Inc. <br />contained favorable recommendations from the Department of Code <br />Enforcement, Bureau of Traffic and Lighting and the Fire Department. <br />It was noted that this was a renewal application. <br />The Rubbish and Garbage application of Fred Stepaniak, for P & R <br />Trash, indicated that an inspection of the truck to be used had <br />been conducted by the Solid Waste Bureau and was found to be in <br />compliance. <br />The Public Parking application of Robert Williams for Rob's <br />Parking contained the required Proof of Insurance as well as the <br />hours and rates to be charged. <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and <br />carried, the above applications were approved and referred to <br />the Deputy Controller's office for issuance. <br />APPROVE RELEASE OF CONTRACTOR'S BOND. <br />Mr. Ray S. Andrysiak, P.E., Engineering Department, recommended <br />that the Contractor's Bond for Hubert Woodcox, d/b/a Woodcox <br />Building be released effective April 16, 1984. Upon a motion <br />made by Mr. Leszczynski, seconded by Mr. Vance and carried, the <br />above Contractor's Bond was released. <br />APPROVE CONTRACTOR'S BOND <br />Mr. Ray S. Andrysiak, P.E., Engineering Department, recommended <br />that the Contractor's Bond for Woodcox Building Contractor, Inc. <br />be approved effective April 16, 1984. Upon a motion made by Mr. <br />Leszczynski, seconded by Mr. Vance and carried, the above <br />Contractor's Bond was approved. <br />APPROVE HANDICAPPED PARKING PERMITS <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and <br />carried, the following eight (8) handicapped parking permit <br />applications were approved and referred to the Deputy Controller's <br />office for issuance of permits: <br />Reedie Cogdell, 214 S.
