74, - REGULAR MEETING APRIL 16, 1984 APPROVE LICENSE APPLICATIONS Mr. Leszczynski advised that the following license applications have been received: MASSAGE TECHNICIAN: MASSAGE ESTABLISHMENT RUBBISH & GARBAGE: PUBLIC PARKING FACILITY: Berthella Stevens 69901 Elm Road Bremen, Indiana L'Image Classique, 519 East Jefferson South Bend, Indiana By: E., Erika Mays Fred Stepaniak P & R Trash 51981 U.S. 31 North South Bend, Indiana Inc. Boulevard President Robert_ Williams Rob' s Parking 120 West.Western Avenue South Bend, Indiana Mr. Leszczynski further advised that the Massage Technician applica- tion of Berthella Stevens contained the necessary doctor's certificate and received a favorable recommendation from the Police Department. The Massage Establishment application of L'Image Classique, Inc. contained favorable recommendations from the Department of Code Enforcement, Bureau of Traffic and Lighting and the Fire Department. It was noted that this was a renewal application. The Rubbish and Garbage application of Fred Stepaniak, for P & R Trash, indicated that an inspection of the truck to be used had been conducted by the Solid Waste Bureau and was found to be in compliance. The Public Parking application of Robert Williams for Rob's Parking contained the required Proof of Insurance as well as the hours and rates to be charged. Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and carried, the above applications were approved and referred to the Deputy Controller's office for issuance. APPROVE RELEASE OF CONTRACTOR'S BOND. Mr. Ray S. Andrysiak, P.E., Engineering Department, recommended that the Contractor's Bond for Hubert Woodcox, d/b/a Woodcox Building be released effective April 16, 1984. Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and carried, the above Contractor's Bond was released. APPROVE CONTRACTOR'S BOND Mr. Ray S. Andrysiak, P.E., Engineering Department, recommended that the Contractor's Bond for Woodcox Building Contractor, Inc. be approved effective April 16, 1984. Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and carried, the above Contractor's Bond was approved. APPROVE HANDICAPPED PARKING PERMITS Upon a motion made by Mr. Leszczynski, seconded by Mr. Vance and carried, the following eight (8) handicapped parking permit applications were approved and referred to the Deputy Controller's office for issuance of permits: Reedie Cogdell, 214 S.
