Pfotenhauer advised that the bid <br />submitted by Casteel Construction Corporation, 23086 West Ireland <br />Road, P.O. Box 628, South Bend, Indiana, contained a fatal defect <br />in that the Non -Collusion Affidavit was not signed and therefore <br />should be rejected. Upon a motion made by Mr. Vance, seconded by <br />Ms. Barnard and carried, the bid was rejected for non-compliance <br />with bid requirements. <br />Upon a motion made by Mr. Leszczynski, seconded by Ms. Barnard <br />and carried, the bids were referred to the Engineering Department <br />and James E. Childs & Associates, for review and recommendation. <br />APPROVE COMMUNITY DEVELOPMENT CONTRACT AND ADDENDA <br />The following Community Development Contract and Addenda were <br />presented to the Board for approval: <br />NEIGHBORHOOD HOUSING SERVICES OF SOUTH BEND, INC. <br />NHS REVOLVING LOAN/NEW CONSTRUCTION FUND <br />The targeted goals for the NHS Revolving Loan Activity are to <br />construct one (1) new housing unit and provide six (6) loans <br />to eligible applicants. The total cost of this activity <br />shall not exceed $100,000.00. No more than $45,000.00 <br />may be used for the new construction project. <br />ADDENDUM I <br />NEIGHBORHOOD CODE ENFORCEMENT OFFICE <br />DEMOLITION PROGRAM <br />Addendum I provides for the addition of $25,000.00 to the <br />Contract budget. The total cost of the demolition program <br />shall not exceed $65,000.00. <br />ADDENDUM IV <br />DEPARTMENT OF PUBLIC WORKS <br />HANDICAPPED ACCESS CURB CUTS PROJECT <br />Addendum IV provides for the reduction of the budget by <br />$21,657.00 and moved the activity to General Street <br />Improvements. The total cost of this activity shall not <br />exceed $69,087.00 <br />ADDENDUM III <br />DEPARTMENT OF PUBLIC WORKS <br />SPECIAL PUBLIC WORKS ACTIVITY <br />Addendum III provides for the addition of $40,131.00 to the <br />budget for this activity and adds L.W.W. Curb and Sidewalk <br />Improvements as an eligible activity. The total cost of this <br />activity shall not exceed $40,131.00 <br />ADDENDUM I <br />NEIGHBORHOOD CODE ENFORCEMENT <br />CODE ENFORCEMENT REHABILITATION FUND ACTIVITY <br />Addendum I provides for a reduction in the budget for this <br />activity making the new contract amount $15,000.00. <br />Ll <br />
