Taylor submitted a <br />Medicaid Provider Agreement which will re -enroll the South Bend <br />Fire Department as a provider of services to Indiana Medicaid <br />recipients. Chief Taylor further advised that this Agreement is <br />similar to one executed by the Board last year and is required <br />annually. Upon a motion made by Mr. Leszczynski, seconded by <br />Mrs. DeClercq and carried, the Agreement as submitted was <br />approved and executed. <br />ACCEPTANCE AND APPROVAL OF WARRANTY DEED - SAMPLE STREET WIDENING <br />PROJECT <br />Mr. Leszczynski advised that the Deed submitted to the Board <br />today for acceptance and approval is for the property located at <br />the northeast corner of Sample and Walnut Streets. It was noted <br />that when property was acquired in 1980 for the Sample Street <br />Widening Project this Deed was inadvertently misplaced and not <br />submitted to the Board for acceptance and approval. It was <br />further noted that this property is part of a Lot deeded to <br />Stanley Kotolinski and Wanda Kotolinski, also known as Lot 18 as <br />shown on the recorded plat of Honer's Subdivision in Plat Book 3, <br />Page 53, of a part of Bank Out Lot Numbered one hundred one (101) <br />of the Second Plat of Out Lots of the Town, now City of South <br />Bend, platted by the State Bank of the State of Indiana. Upon a <br />motion made by Mr. Leszczynski, seconded by Mrs. DeClercq and <br />carried, the Warranty Deed as submitted was accepted. <br />AWARD BIDS - GASOLINE AND FUEL OIL <br />Mr. Phil St. Clair, Director, Division of Equipment Services, <br />advised the Board that he has reviewed the bids received on <br />July 28, 1987 and recommends the following awards be made: <br />TANKWAGON DELIVERIES - ALL LOCATIONS: <br />Regular gasoline <br />Berreth Oil Company <br />$0.660/gal. <br />Unleaded gasoline <br />Berreth Oil Company <br />$0.645/gal. <br />Diesel Fuel #1 <br />Berreth Oil Company <br />$0.644/gal. <br />Diesel Fuel #2 <br />Berreth Oil Company <br />$0.604/gal. <br />BULK TRANSPORT DELIVERIES - ALL LOCATIONS: <br />Regular gasoline Berreth Oil Company $0.620/gal. <br />Unleaded gasoline Berreth Oil Company $0.605/gal. <br />Diesel Fuel #1 Petroleum Traders $0.5988/gal. <br />Diesel Fuel #2 Petroleum Traders $0.5539/gal. <br />Upon a motion made by Mrs. DeClercq, seconded by Mr. Leszczynski <br />and carried, the recommendation was accepted and the bids <br />awarded. <br />ADOPTION OF RESOLUTION NO. 23-1987 - ESTABLISHING RECORDING FEE <br />(SALE OF CITY -OWNED PROPERTY) <br />Mr. Leszczynski advised that Resolution No. 23-1987 being <br />submitted today would allow the Clerk of the Board to collect a <br />$5.00 recording fee in addition to the amount offered for the <br />purchase of city -owned property. <br />
