Taylor submitted a Medicaid Provider Agreement which will re -enroll the South Bend Fire Department as a provider of services to Indiana Medicaid recipients. Chief Taylor further advised that this Agreement is similar to one executed by the Board last year and is required annually. Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq and carried, the Agreement as submitted was approved and executed. ACCEPTANCE AND APPROVAL OF WARRANTY DEED - SAMPLE STREET WIDENING PROJECT Mr. Leszczynski advised that the Deed submitted to the Board today for acceptance and approval is for the property located at the northeast corner of Sample and Walnut Streets. It was noted that when property was acquired in 1980 for the Sample Street Widening Project this Deed was inadvertently misplaced and not submitted to the Board for acceptance and approval. It was further noted that this property is part of a Lot deeded to Stanley Kotolinski and Wanda Kotolinski, also known as Lot 18 as shown on the recorded plat of Honer's Subdivision in Plat Book 3, Page 53, of a part of Bank Out Lot Numbered one hundred one (101) of the Second Plat of Out Lots of the Town, now City of South Bend, platted by the State Bank of the State of Indiana. Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq and carried, the Warranty Deed as submitted was accepted. AWARD BIDS - GASOLINE AND FUEL OIL Mr. Phil St. Clair, Director, Division of Equipment Services, advised the Board that he has reviewed the bids received on July 28, 1987 and recommends the following awards be made: TANKWAGON DELIVERIES - ALL LOCATIONS: Regular gasoline Berreth Oil Company $0.660/gal. Unleaded gasoline Berreth Oil Company $0.645/gal. Diesel Fuel #1 Berreth Oil Company $0.644/gal. Diesel Fuel #2 Berreth Oil Company $0.604/gal. BULK TRANSPORT DELIVERIES - ALL LOCATIONS: Regular gasoline Berreth Oil Company $0.620/gal. Unleaded gasoline Berreth Oil Company $0.605/gal. Diesel Fuel #1 Petroleum Traders $0.5988/gal. Diesel Fuel #2 Petroleum Traders $0.5539/gal. Upon a motion made by Mrs. DeClercq, seconded by Mr. Leszczynski and carried, the recommendation was accepted and the bids awarded. ADOPTION OF RESOLUTION NO. 23-1987 - ESTABLISHING RECORDING FEE (SALE OF CITY -OWNED PROPERTY) Mr. Leszczynski advised that Resolution No. 23-1987 being submitted today would allow the Clerk of the Board to collect a $5.00 recording fee in addition to the amount offered for the purchase of city -owned property.
