0 <br />REGULAR MEETING <br />JANUARY 20, 1987 <br />$18,000.00 in 1986. The Proposal submitted by Madison Center for <br />the period January 1, 1987 to December 31, 1987, would cost <br />$13,500.00 and provides the following: <br />- Development and training including brochures explaining <br />the program to all employees, follow-up letters, I.D. <br />cards, orientation sessions explaining benefits to <br />employees, and special enhancement programs for employees <br />on stress management, single parenting, communications <br />in the workplace and other subjects as requested. <br />Reporting and follow-up documenting utilization of <br />services and monitoring on the condition and progress of <br />employees in the program. <br />Assessment and treatment of individual problems with the <br />first six (6) visits per employee covered as part of the <br />City's fee. Additional charges will then be billed against <br />the employees insurance. <br />Mr. Vance stated that the program as presented by Madison Center <br />is considerably more expansive in scope than the current services <br />being offered and the reporting and feedback as outlined by <br />Madison Center is greater in scope in addition to the yearly cost <br />being $5,000.00 lower. He therefore, also recommended that the <br />City enter into the Proposal with Madison Center. <br />Mrs. DeClercq, based on the recommendations received and the <br />benefits of the Proposal, made a motion that the Proposal with <br />Madison Center as outlined above be approved. Mr. Leszczynski <br />seconded the motion, it carried, and the Proposal was approved <br />and executed. <br />APPROVE COMMUNITY DEVELOPMENT ADDENDA <br />The following Community Development Addenda were presented to the <br />Board for approval: <br />ADDENDUM I <br />DEPARTMENT OF PUBLIC WORKS <br />MONROE PARK IMPROVEMENTS <br />Addendum I provides for a time extension from December 31, <br />1986 to December 31, 1987. Additionally, Addendum I adds <br />landscaping as an eligible activity and appraisals as an <br />eligible cost. <br />ADDENDUM III <br />SOUTH BEND HERITAGE FOUNDATION <br />NEAR WEST SIDE COMMERCIAL LOAN ACTIVITY <br />Addendum III provides for a time extension until March <br />31, 1987. <br />Upon a motion made by Mrs. DeClercq, seconded by Mr. Leszczynski <br />and carried, the above Addenda were approved and executed. <br />APPROVE AGREEMENT FOR CONSULTING SERVICES - NEIGHBORHOOD BUSINESS <br />DISTRICTS <br />Ms. Betsy Harriman, Office of Community Development, advised the <br />Board that the above referred to Agreement with The Chesapeake <br />Group, Inc., Baltimore, Maryland, provides technical assistance <br />to the City in revitalizing three (3) designated neighborhood <br />business districts (Western Avenue, Lincolnway West and South <br />Gateway). The Chesapeake Group, Inc. shall be paid the sum of <br />$30,000.00 for said services. Upon a motion made by Mrs. <br />DeClercq, seconded by Mr. Leszczynski and carried, the above <br />referred to Agreement was approved and executed. <br />1 <br />1 <br />1 <br />
