REGULAR MEETING SEPTEMBER 17, 1990 <br />APPROVE RAILROAD AGREEMENTS: <br />RECONSTRUCTION OF THE FORD STREET GRAND TRUNK WESTERN GRADE <br />CROSSING <br />RECONSTRUCTION OF THE 30TH STREET GRAND TRUNK WESTERN GRADE <br />CROSSING <br />Mrs. DeClercq advised that the Board is in receipt of Railroad <br />Agreements as submitted by Mr. R. E. Wheeler, Area Engineer, <br />Division of Local Assistance, Indiana Department of <br />Transportation, 100 North Senate Avenue, Room 1101, Indianapolis, <br />Indiana, in regards to the above referred to locations. <br />Therefore, Mrs. DeClercq made a motion that the Agreements be <br />approved and executed. Ms. Humphreys seconded the motion which <br />carried. <br />APPROVE NEONATAL AMBULANCE LEASE AND STAFFING AGREEMENT <br />MEMORIAL HOSPITAL <br />In a letter to the Board, Fire Chief Luther Taylor submitted a <br />renewal Neonatal Ambulance Agreement between the City and <br />Memorial Hospital. Chief Taylor stated that this Agreement has <br />been in operation by the South Bend Fire Department for <br />approximately one (1) year and is working fine for the City and <br />the hospital. He also noted that during this past year, the <br />Department responded to approximately one hundred fifty-five <br />(155) calls with this specialized unit, which was within their <br />projections. After evaluating the revenue generated from this <br />unit, Chief Taylor stated that he believes it to be to the City's <br />advantage to continue this Agreement. <br />It is noted that the Agreement, which shall be in effect until <br />December 31, 1991, reflects a ten per cent (10a) increase over <br />the previous Agreement and indicates that Memorial Hospital shall <br />pay to the City the sum of $3,025.00 per month to assure <br />adequate staffing to meet the lowest possible response time. <br />Therefore, Mrs. DeClercq made a motion that the Agreement as <br />submitted be approved and executed. Ms. Humphreys seconded the <br />motion which carried. <br />APPROVE CHANGE ORDER NO. 2 - VIEWING PARK PROJECT <br />Mrs. DeClercq advised that Kaser-Spraker Construction, Inc., P.O. <br />Box 3605, South Bend, Indiana, has submitted Change Order No. 2 <br />indicating that the Contract amount be decreased by $1,031.00 for <br />a new Contract sum including this Change Order in the amount of <br />$296,868.00. Upon a motion made by Ms. Humphreys, seconded by <br />Mrs. DeClercq and carried, the Change Order as submitted was <br />approved and executed. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - NILES AVENUE PHASE II <br />PROJECT <br />Mrs. DeClercq advised that Walsh & Kelly, Inc., 24358 State Road <br />23, South Bend, Indiana, has submitted the Project Completion <br />Affidavit for the above referred to project indicating a final <br />cost of $400,558.06. Also submitted was the required three (3) <br />year Maintenance Bond. Mrs. DeClercq made a motion that the <br />Project Completion Affidavit be approved and executed and the <br />Maintenance Bond be accepted for filing. Ms. Humphreys seconded <br />the motion which carried. <br />APPROVE REQUEST OF THE SOUTH BEND REDEVELOPMENT COMMISSION THAT <br />THE BOARD OF PUBLIC WORKS ACT AS ITS REPRESENTATIVE FOR THE <br />DESIGN AND CONSTRUCTION OF PUBLIC IMPROVEMENTS IN THE AIRPORT <br />ECONOMIC DEVELOPMENT AREA <br />In a letter to the Board, Ms. Ann E. Kolata, Deputy Executive <br />Director, Department of Economic Development, advised that the <br />South Bend Redevelopment Commission requests that the Board of <br />
