REGULAR MEETING SEPTEMBER 17, 1990 APPROVE RAILROAD AGREEMENTS: RECONSTRUCTION OF THE FORD STREET GRAND TRUNK WESTERN GRADE CROSSING RECONSTRUCTION OF THE 30TH STREET GRAND TRUNK WESTERN GRADE CROSSING Mrs. DeClercq advised that the Board is in receipt of Railroad Agreements as submitted by Mr. R. E. Wheeler, Area Engineer, Division of Local Assistance, Indiana Department of Transportation, 100 North Senate Avenue, Room 1101, Indianapolis, Indiana, in regards to the above referred to locations. Therefore, Mrs. DeClercq made a motion that the Agreements be approved and executed. Ms. Humphreys seconded the motion which carried. APPROVE NEONATAL AMBULANCE LEASE AND STAFFING AGREEMENT MEMORIAL HOSPITAL In a letter to the Board, Fire Chief Luther Taylor submitted a renewal Neonatal Ambulance Agreement between the City and Memorial Hospital. Chief Taylor stated that this Agreement has been in operation by the South Bend Fire Department for approximately one (1) year and is working fine for the City and the hospital. He also noted that during this past year, the Department responded to approximately one hundred fifty-five (155) calls with this specialized unit, which was within their projections. After evaluating the revenue generated from this unit, Chief Taylor stated that he believes it to be to the City's advantage to continue this Agreement. It is noted that the Agreement, which shall be in effect until December 31, 1991, reflects a ten per cent (10a) increase over the previous Agreement and indicates that Memorial Hospital shall pay to the City the sum of $3,025.00 per month to assure adequate staffing to meet the lowest possible response time. Therefore, Mrs. DeClercq made a motion that the Agreement as submitted be approved and executed. Ms. Humphreys seconded the motion which carried. APPROVE CHANGE ORDER NO. 2 - VIEWING PARK PROJECT Mrs. DeClercq advised that Kaser-Spraker Construction, Inc., P.O. Box 3605, South Bend, Indiana, has submitted Change Order No. 2 indicating that the Contract amount be decreased by $1,031.00 for a new Contract sum including this Change Order in the amount of $296,868.00. Upon a motion made by Ms. Humphreys, seconded by Mrs. DeClercq and carried, the Change Order as submitted was approved and executed. APPROVE PROJECT COMPLETION AFFIDAVIT - NILES AVENUE PHASE II PROJECT Mrs. DeClercq advised that Walsh & Kelly, Inc., 24358 State Road 23, South Bend, Indiana, has submitted the Project Completion Affidavit for the above referred to project indicating a final cost of $400,558.06. Also submitted was the required three (3) year Maintenance Bond. Mrs. DeClercq made a motion that the Project Completion Affidavit be approved and executed and the Maintenance Bond be accepted for filing. Ms. Humphreys seconded the motion which carried. APPROVE REQUEST OF THE SOUTH BEND REDEVELOPMENT COMMISSION THAT THE BOARD OF PUBLIC WORKS ACT AS ITS REPRESENTATIVE FOR THE DESIGN AND CONSTRUCTION OF PUBLIC IMPROVEMENTS IN THE AIRPORT ECONOMIC DEVELOPMENT AREA In a letter to the Board, Ms. Ann E.
