135 <br />REGULAR MEETING <br />APRIL 23, 1990 <br />H.B. 1240 - recently passed mandates a $.50 per ton fee on all <br />wastes disposed of in the state of Indiana effective 1/1/91. <br />This fee will be passed on at that time. Any ad all fees <br />imposed by local or state governments will be passed on. <br />BID PROPOSAL FOR RECYCLING BOXES <br />Hauling Charge Per load $120.00 <br />Disposal Charge -0- <br />Container Rental Fee per month $ 60.00 <br />The above rates for recycling reflect the City of South Bend <br />receiving one hundred per cent (1000) of the revenues generated <br />from the sale of the material. Revenue is based on current <br />markets. <br />Method of Disposal: All waste collected and transported from <br />the City of South Bend would be disposed using the sanitary <br />landfill method. All waste collected will be disposed of at <br />the following licensed sanitary landfill: Prairie View <br />Landfill, 15505 Shiverly Road, Wyatt, Indiana. <br />Recycling Materials: All recyclables collected and <br />transported from the City of South Bend will be purchased, <br />processed and sold at the following recycling buy back center: <br />South Bend Waste Paper, 1519 South Franklin, South Bend, <br />Indiana. <br />Upon a motion made by Mr. Leszczynski, seconded by Ms. Humphreys <br />and carried, the bids were referred to the Department of <br />Sanitation for review and recommendation and the <br />Non -Discrimination Commitment forms as submitted with the bids <br />were accepted for filing. <br />OPENING OF BIDS -LIFEPAK 5 MONITORS/DEFIBRILLATOR <br />Mr. Leszczynski advised that pursuant to the adoption of <br />Resolution No. 11-1990 declaring obsolete Fire Department Lifepak <br />Monitors and Defibrillators, the equipment was offered for sale <br />with today being the date for which sealed bids were to be <br />submitted. The following sealed bids were received: <br />UNIVERSITY OF NOTRE DAME <br />Notre Dame, Indiana <br />Sealed bid was submitted by Ms. Carol Seager, Director, <br />University Health Services. <br />BID: $2,000.00 for the following equipment: <br />Lifepak 5 Monitor Serial #0014920 (1982) <br />Defibrillator Unit Serial #00014384 <br />SAINT JOSEPH'S MEDICAL CENTER <br />801 East LaSalle <br />P.O. Box 1935 <br />South Bend, Indiana 46634-1935 <br />Bid was submitted by Mr. Janet L. Balog, R.N., Administrator, <br />Emergency and Community Health Resources. <br />BID: $2,001.00 for either of the two (2) <br />monitor/defibrillators available for purchase <br />Upon a motion made by Mr. Leszczynski, seconded by Ms. Humphreys <br />and carried, the bids were referred to the Fire Department for <br />review and recommendation. <br />
