Klosinski <br />SUBJECT: Agenda Request Form — Change Order No. 7 Final; Project Completion <br />Affidavit <br />Project No. 111-071 Wastewater Treatment Plant Primary Clarifier <br />Rehabilitation and Equipment Upgrades <br />DATE: September 2, 2016 <br />The Division of Utilities is submitting Change Order No. 7 and the Project Completion Affidavit <br />for Project No. 111-071 Primary Clarifier Rehabilitation and Equipment Upgrades for review and <br />approval. <br />Change Order No. 7 includes the following items: <br />• Deductive Backcharge for Primary Clarifier Tank 5 ($40,000) <br />Total Net Change <br />($40,000) <br />This change order captures the adjustments to the contract price for delays associated with <br />Primary Clarifier Tank 5 construction dating back to 2013-2014. The Contractor has completed <br />all construction. The Division of Utilities recommends that the Board accepts the deductive <br />backcharge amount (resulting in a deduction to the final project cost). <br />The Project Completion Affidavit is also submitted for the Board of Public Works approval. <br />This project is Division of Environmental Services account number 642-0630-415-43-66 <br />(Wastewater Capital Fund, Project Code 13T001). <br />If there are any questions regarding the Agenda Request, please contact Al Greek (Ext. 4210) <br />or Jacob M.
