REGULAR MEETING <br />7ULY 29, 1991 <br />Inc., be approved effective July 29, 1991. Mr. Leszczynski made <br />a motion that the recommendation be accepted and the bond be <br />approved. Ms. Humphreys seconded the motion which carried. <br />APPROVE TRAFFIC CONTROL DEVICE <br />Upon a motion made by Ms. Humphreys, seconded by Mr. Leszczynski <br />and carried, the following one (1) traffic control device was <br />approved: <br />1, NEW INSTALLATION: No Parking <br />12:00 noon to 7:00 p.m. on <br />August 16, 1991 <br />LOCATION: On Jefferson Boulevard and <br />Lafayette Boulevard around the <br />County -City Building <br />REMARKS: For Health Department Measles <br />Clinic. <br />APPROVE SIDEWALK SALE PERMIT APPLICATION <br />Mr. Leszczynski advised that the Board is in receipt of a <br />Sidewalk Sale Permit application from the following: <br />NAME: MS. SHOPPE <br />ADDRESS: 106 Wayne Street <br />DATES: August 1-2-3, 1991 <br />BY: Geneva Foulks <br />Mr. Leszczynski noted that the required Indemnification and Hold <br />Harmless Agreement was completed and attached to the <br />application. Therefore, Mr. Leszczynski made a motion that the <br />application be approved. Ms. Humphreys seconded the motion which <br />carried. <br />APPROVE LICENSE APPLICATION - RUBBISH AND GARBAGE REMOVAL <br />Mr. Leszczynski advised that the following one (1) Rubbish and <br />Garbage Removal license application has been received: <br />1, Powell Highsmith <br />a806 Linden Avenue <br />South Bend, Indiana <br />It was noted that an inspection of the truck to be used was <br />conducted by the Solid Waste Bureau and the applicant was found <br />to be in compliance. Upon a motion made by Ms. Humphreys, <br />seconded by Mr. Leszczynski and carried, the above license <br />application was approved and referred to the Deputy Controller's <br />office for issuance of a license. <br />FILING OF ENVIRONMENTAL CLEAN-UP OF LOTS REPORTS <br />Mr. Leszczynski advised that the Board is in receipt of two (2) <br />lists from the Department of Code Enforcement indicating that <br />fifty-seven (57) miscellaneous and twenty-one (21) City <br />owned/miscellaneous properties were cleaned from July 15, 1991 to <br />July 18, 1991. Mr. Leszczynski made a motion that the list as <br />submitted be filed. Ms. Humphreys seconded the motion which <br />carried. <br />APPROVE CLAIMS <br />Chief Deputy City Controller John D. Leisenring, submitted Claim <br />Docket Nos. 8619, 8620, 8901 and 8902 as well as 8930 through <br />9171 and recommended approval. Ms. Humphreys made a motion that <br />the claims be approved and the reports filed. Mr. Leszczynski <br />seconded the motion which carried. <br />
