REGULAR MEETING 7ULY 29, 1991 Inc., be approved effective July 29, 1991. Mr. Leszczynski made a motion that the recommendation be accepted and the bond be approved. Ms. Humphreys seconded the motion which carried. APPROVE TRAFFIC CONTROL DEVICE Upon a motion made by Ms. Humphreys, seconded by Mr. Leszczynski and carried, the following one (1) traffic control device was approved: 1, NEW INSTALLATION: No Parking 12:00 noon to 7:00 p.m. on August 16, 1991 LOCATION: On Jefferson Boulevard and Lafayette Boulevard around the County -City Building REMARKS: For Health Department Measles Clinic. APPROVE SIDEWALK SALE PERMIT APPLICATION Mr. Leszczynski advised that the Board is in receipt of a Sidewalk Sale Permit application from the following: NAME: MS. SHOPPE ADDRESS: 106 Wayne Street DATES: August 1-2-3, 1991 BY: Geneva Foulks Mr. Leszczynski noted that the required Indemnification and Hold Harmless Agreement was completed and attached to the application. Therefore, Mr. Leszczynski made a motion that the application be approved. Ms. Humphreys seconded the motion which carried. APPROVE LICENSE APPLICATION - RUBBISH AND GARBAGE REMOVAL Mr. Leszczynski advised that the following one (1) Rubbish and Garbage Removal license application has been received: 1, Powell Highsmith a806 Linden Avenue South Bend, Indiana It was noted that an inspection of the truck to be used was conducted by the Solid Waste Bureau and the applicant was found to be in compliance. Upon a motion made by Ms. Humphreys, seconded by Mr. Leszczynski and carried, the above license application was approved and referred to the Deputy Controller's office for issuance of a license. FILING OF ENVIRONMENTAL CLEAN-UP OF LOTS REPORTS Mr. Leszczynski advised that the Board is in receipt of two (2) lists from the Department of Code Enforcement indicating that fifty-seven (57) miscellaneous and twenty-one (21) City owned/miscellaneous properties were cleaned from July 15, 1991 to July 18, 1991. Mr. Leszczynski made a motion that the list as submitted be filed. Ms. Humphreys seconded the motion which carried. APPROVE CLAIMS Chief Deputy City Controller John D. Leisenring, submitted Claim Docket Nos. 8619, 8620, 8901 and 8902 as well as 8930 through 9171 and recommended approval. Ms. Humphreys made a motion that the claims be approved and the reports filed. Mr. Leszczynski seconded the motion which carried.
