M- <br />REGULAR MEETING <br />JULY 1, 1991 <br />Upon a motion made by Mr. Leszczynski, seconded by Ms. Humphreys <br />and carried, the above requests were referred to the appropriate <br />City departments and bureaus for review and recommendation. <br />APPROVE SIDEWALK SALE PERMIT APPLICATION <br />Mr. Leszczynski noted that the following Sidewalk Sale Permit <br />Application has been submitted to the Board for approval: <br />NAME: Brant's Ben Franklin <br />BY: Ray Brant <br />ADDRESS: 2114 Miami Street <br />DATES: August 1-3, 1991 <br />Mr. Leszczynski noted that the required Indemnification Agreement <br />was executed by Mr. Brant and was attached to the application. <br />Ms. Humphreys made a motion that the application_ be approved. <br />Mr. Leszczynski seconded the motion which carried. <br />APPROVE REQUESTS: <br />- REQUEST OF CITY AWNING TO INSTALL• CANOPY AT 1510 SOUTH <br />MICHIGAN STREET - PDH OFFICE PRODUCTS <br />REQUEST OF ST. STEPHEN'S PARISH TO CLOSE MC PHERSON STREET, <br />FROM THOMAS TO NAPIER AND TO CONDUCT 5K RUN IN CONJUNCTION <br />WITH PARISH FESTIVAL - AUGUST 10-11, 1991 <br />- REQUEST TO CLOSE FREDRICKSON, FROM JOHNSON TO O'BRIEN, FOR <br />BLOCK PARTY - JULY 4, 1991 <br />Mr. Leszczynski advised that the appropriate City departments and <br />bureaus have reviewed the above referred to requests which were <br />submitted to the Board on June 17, 1991, and approval is <br />recommended. Therefore, Ms. Humphreys made a motion that the <br />recommendations be accepted and the -requests be approved. Mr. <br />Leszczynski seconded the motion which carried. <br />APPROVE TITLE SHEET - POINTE VIRIDIAN <br />Mr, Leszczynski indicated that the Title Sheet for the above <br />referred to project was being presented at this time for <br />execution_. Upon a motion made by Ms. Humphreys, seconded by Mr. <br />Leszczynski and carried, the above referred to Title Sheet was <br />approved and signed. <br />APPROVE CONTRACTORS AND EXCAVATION BONDS <br />Mr. Leszczynski stated that the Engineering <br />recommended that the following Contractors <br />be approved as indicated: <br />CONTRACTORS BONDS <br />Tim Hoover, d/b/a Approved <br />Hoover Concrete <br />1507 Portage Road <br />South Bend, Indiana <br />Troyer Construction Approved <br />EXCAVATION BOND <br />Department has <br />and Excavation Bonds <br />Effective 7-1-91 <br />Effective 7-1-91 <br />- Troyer Construction Approved Effective 7-1-91 <br />Mr. Leszczynski made a motion that the recommendations be <br />accepted and the bonds be approved. Ms. Humphreys seconded the <br />motion which carried. <br />
