REGULAR MEETING <br />JUNE 3, 1991 <br />FONTAINE TRUCK EQUIPMENT COMPANY <br />2770 Bluff Road <br />Indianapolis, Indiana <br />Bid was signed by Mr. George Wilson, Assistant General <br />Manager, Non -Collusion Affidavit was in order, Non - <br />Discrimination Commitment form was completed and a Bid <br />Bond in the amount of $4,800.00 was submitted. <br />BID: <br />Twelve (12) Dump Bodies FTEC B10U <br />Each: $ 3,453.00 <br />Total: $41,436.00 <br />NORTHERN EQUIPMENT COMPANY, INC. <br />16465 West Lincoln Highway <br />Plymouth, Indiana 46563 <br />Bid was -signed by Mr. Willard Frost, President, Non - <br />Collusion Affidavit was in order, Non -Discrimination <br />Commitment form was completed and a Certified Check <br />in the amount of $4,326.00 was submitted. <br />BID• <br />Twelve (12) Galion, Model 408-U, 10' dump bodies with <br />Model U-850-C hoists, 1/2 cab shields, 1/2" x3" flat <br />steel re-enforcements, to meet all specifications. All <br />lighting to meet all specifications and mounted to meet <br />all specifications. Delivered to the City of South Bend: <br />Total: $43,260.00 <br />Upon a motion made by Mr. Leszczynski, seconded by Mrs. DeClercq <br />and carried, the bids received were referred to Mr. Phil St. <br />Clair, Director, Division of Equipment Services, for review and <br />recommendation. <br />OPENING OF PROPOSALS - DEBT COLLECTION SERVICES FOR PAST DUE <br />,AMBULANCE ACCOUNTS FOR THE SOUTH BEND FIRE DEPARTMENT FOR 1991 <br />This was the date set for receiving and opening of sealed <br />Proposals for the above referred to service. The Clerk tendered <br />proofs of publication of Notice in the South Bend Tribune and the <br />Tri-County News which were found to be sufficient. The following <br />bids were opened and publicly read: <br />,:REDIT BUREAU OF SOUTH BEND-MISHAWAKA <br />312 West Colfax Avenue <br />South Bend, Indiana 46634 <br />Proposal was submitted by Mr. Ron R. Wujcik, General <br />Manager. <br />PROPOSAL: <br />Commission rate is thirty per cent (300) for accounts with <br />the exception of accounts placed in suit or forwarded, which <br />is fifty per cent ( 50 0) . <br />FEDERATED COLLECTION AGENCY <br />2015 West Western Avenue <br />Suite 138, Marycrest Building <br />South Bend, Indiana 46629 <br />Proposal was submitted by Mr. Robert Forgash, President. <br />PROPOSAL: <br />Federated Shall retain thirty-five per cent (35%) for all <br />monies collected and no fees will be assessed on unrecovered <br />1 <br />1 <br />1 <br />accounts. <br />
