<br />APPROVE CONSTRUCTION CONTRACT - ASBES
<br />1302-1304-1/2 WEST WASHINGTON PROJECT

<br />Mr. Leszczynski advised that in accordance with the bid awarded
<br />on February 18, 1991, to Specialty Systems of Illinois, Inc.,
<br />South Bend Division, 1408 Elwood Avenue, South Bend, Indiana, in
<br />the amount of $7,380.37, for the above referred to project, a
<br />Contract in said amount was submitted to the Board for approval.
<br />Upon a motion made by Mrs. DeClercq, seconded by Ms. Humphreys
<br />and carried, the Contract was approved and the appropriate
<br />Performance Bond, Labor and Material Payment Bond and Certificate
<br />of Insurance as submitted were filed.
<br />APPROVE CONSTRUCTION CONTRACT - TWO (2) 10,000 GALLON UNDERGROUND
<br />MOTOR FUEL STORAGE TANKS, REMOVAL OF OLD TANKS AND DISPENSING
<br />EQUIPMENT INSTALLATION - WASTEWATER TREATMENT PLANT
<br />Mr. Leszczynski advised that in accordance with the bid awarded
<br />on April 8, 1991, to Petroleum Equipment, Inc., 12875 McKinley
<br />Highway, Mishawaka, Indiana, in the amount of $68,981.49 for the
<br />above referred to project, a Contract in said amount was
<br />submitted to the Board for approval. Upon a motion made by Ms.
<br />Humphreys, seconded by Mrs. DeClercq and carried, the Contract
<br />was approved and the appropriate Performance Bond and Labor and
<br />Material Payment Bond as submitted were filed.
<br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION
<br />AFFIDAVIT - NILES AVENUE PHASE IV PROJECT
<br />Mr. Leszczynski advised that Rieth-Riley Construction Company,
<br />Inc., 25200 S.R. 23, South Bend, Indiana, has submitted Change
<br />Order No. 1 (Final) indicating that the Contract amount for the
<br />above referred to project be increased by $10,469.15 for a new
<br />Contract sum including this Change Order in the amount of
<br />$546,593.70. Additionally pubmitted was the Project Completion
<br />Affidavit indicating this new final cost. Upon a motion made by
<br />Mr. Leszczynski, seconded by Mrs. DeClercq and carried, Change
<br />Order No. 1 (Final) and the Project Completion Affidavit were
<br />approved and the appropriate three-year Maintenance Bond as
<br />submitted was accepted for filing.
<br />APPROVE CHANGE ORDER NO. 3 - REMODELING OF EQUIPMENT SERVICES
<br />FACILITY
<br />Mr. Leszczynski advised that The Troyer Group, Inc., 415
<br />Lincolnway East, Mishawaka, Indiana and L.L. Geans & Sons, Inc.,
<br />56535 Magnetic Drive, Mishawaka, Indiana, have submitted Change
<br />Order No. 3 for the above referred to project indicating an
<br />increase of $13,495.00 for a new Contract sum of $835,976.00.
<br />Mr. Leszczynski noted that this increase is for a number of items
<br />including $11,000.00 to provide 480V, 30 3 wire 400 amp
<br />electrical service. Mr. Leszczynski made a motion that the
<br />Change Order be approved and executed. Mrs. DeClercq seconded
<br />the motion which carried.
<br />AWARD BIDS - FIFTY (50) COMPLETE SURVIVAIR MARK II SELF CONTAINED
<br />BREATHING APPARATUS, 30 MINUTE LP SERIES 9842 UNITS AND FIVE (5)
<br />SURVIVAIR MARK II 60 MINUTE HP, SERIES 9870 UNITS
<br />In a letter to the Board Fire Chief Luther Taylor advised
<br />the South Bend Fire Department offered for sale fifty (50)
<br />(30) minute S.C.B.A.'s and five (5) one (1) hour units. A
<br />that
<br />thirty
<br />provision was inserted requiring all offers to purchase must be
<br />for lots of ten (10) units.
<br />On May 13, 1991, proposals were received and opened by the
<br />Board. The bids on the thirty (30) minute units were as follows:
<br />Warren Township Fire Department 10 units $3,000.00
<br />Penn Township Trustee (Joseph R. Ganser) 10 units $3,000.00
