<br />APPROVE EQUIPMENT LEASE/PURCHASE AGREEMENT, TAX DESIGNATIONS AND
<br />COVENANTS, ESCROW AGREEMENT AND UNIFORM COMMERCIAL CODE FINANCIAL
<br />STATEMENT FORMS UCC-1 IN REGARDS TO FIRE DEPARTMENT AIR TANKS AND
<br />CAB AND CHASSIS
<br />Mr. Leszczynski advised that the above referred to documents have
<br />been submitted to the Board for approval and execution.
<br />It is noted that the Equipment Lease/Purchase Agreement indicates
<br />Chrysler Capital Public Finance Corporation, 106 West llth
<br />Street, Suite 1530, Kansas City, Missouri as the Lessor. A
<br />description of the equipment included in this Lease/Purchase
<br />Agreement is as follows:
<br />- One (1) 1990 Ford 350 LX Modula 144" Super Duty Cab -Chassis
<br />- Ten (10) Survivair 30 minute, 2216 PSI, SCBA, NF with case
<br />- Fifty-five (55) Survivair 30 minute, 2216 PSI, SCBA, less
<br />case
<br />- Six (6) Survivair 60 minute 45000 PSI, SCBA, NFPA less case
<br />- Seventy-one (71) Survivair nosecups
<br />The Agreement also notes that the Rental Payment Amount is
<br />$211,955.46, the interest portion is $25,712.53 and the Principal
<br />portion is $186,242.93.
<br />Upon a motion made by Mrs. DeClercq, seconded by Ms. Humphreys
<br />and carried, the above referred to documents were approved and
<br />executed.
<br />APPROVE REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS - TUB
<br />GRINDER
<br />In a letter to the Board, Mr. Robert Kruszynski, Manager,
<br />Department of Sanitation, requested that the Board advertise for
<br />the receipt of bids for one (1) portable Hammermill grinder,
<br />commonly referred to as a "tub -grinder."
<br />Mr. Kruszynski stated that this unit will be used to process
<br />residential yard waste by pulverization (tree limbs, plywood,
<br />leaves grass clippings, etc.) The unit will be located at the
<br />Organic Resource site on Old Cleveland Road and will be used for
<br />the Yard Waste Program.
<br />Mrs. DeClercq made a motion that the request be approved. Ms.
<br />Humphreys seconded the motion which carried.
<br />AWARD BID - ASBESTOS ABATEMENT - 1217-1221 & 1302-1304-1/2 W.
<br />WASHINGTON STREET
<br />In a Memorandum to the Board, Mr. Bob Allen, Manager, Bureau of
<br />Public Construction, recommended that the Board award the low bid
<br />of Specialty Systems of Illinois, South Bend Division, 1408
<br />Elwood Avenue, South Bend, Indiana, in the amount of $7,380.37
<br />for the above referred to project. Mr. Leszczynski advised that
<br />bids for this project were opened by the Board on February 11,
<br />1991. Upon a motion made by Mrs. DeClercq, seconded by Ms.
<br />Humphreys and carried, the recommendation was accepted and the
<br />bid awarded.
<br />AWARD BID - DEMOLITION - 1215-1/2, 1243 & 1402 W. WASHINGTON
<br />STREET
<br />In a Memorandum to the Board, Mr. Bob Allen, Manager, Bureau of
<br />Public Construction, recommended that the Board award the low bid
<br />of Bradberry Brothers, Inc., 20061 Dice Street, South Bend,
<br />Indiana, in the amount of $12,500.00 for the above referred to
<br />project. Mr. Leszczynski advised that bids for this project were
<br />opened by the Board on February 11, 1991. Upon a motion made by
<br />Mrs. DeClercq, seconded by Ms. Humphreys and carried, the
<br />recommendation was accepted and the bid awarded.
