REGULAR MEETING <br />VEHICLE NO. <br />5. <br />10. <br />RED TAG NO. <br />21852 <br />22183 <br />JUNE 14, 1993 <br />AMOUNT BID <br />$ 22.00 <br />$ 29.95 <br />GRAND TOTAL: $1,120.89 <br />AWARD BID - ONE (1) TYPE I MEDIUM TO HEAVY DUTY MODULAR AMBULANCE <br />In a letter to the Board, Fire Chief Luther Taylor advised that <br />after careful review of the bids received on June 7, 1993, for one <br />(1) medium -heavy duty modular ambulance, the Fire Department <br />recommends that the Board award the bid of Goggan & Associates, <br />Inc., 50811 Killian Lane, Elkhart, Indiana, who represents Horton <br />Emergency Vehicles of Columbus, Ohio. <br />Chief Taylor further advised that in reviewing the bids, Goggan & <br />Associates was the low bidder at $107,950.00. Chief Taylor stated <br />that they are comfortable with the ambulance bid of Goggan & <br />Associates/Horton Emergency Vehicles who have a proven track <br />record in the EMS field. <br />Therefore, Mr. Caldwell made a motion that the recommendation be <br />accepted and the bid be awarded. Mrs. Mueller seconded the motion <br />which carried. <br />CHANGE ORDER NO. 4 (FINAL) - SANITATION <br />Mr. Leszczynski advised that Gibson -Lewis, Inc., 1001 West 11th <br />Street, Mishawaka, Indiana, has submitted Change Order No. 4 <br />(Final) for the above referred to project indicating that the <br />Contract amount be decreased by $2,952.00 for a new Contract sum <br />including this Change Order in the amount of $457,549.00. Upon a <br />motion made by Mr. Caldwell, seconded by Mrs. Mueller and carried, <br />Change Order No. 4 (Final) was approved and executed. <br />APPROVE HOUSE MOVE - FROM THE N.W. CORNER OF CORBY AND HICKORY TO <br />A PARCEL WEST OF 16860 CATALPA <br />In a Memorandum to the Board, Mr. Donald F. Fozo, Building <br />Commissioner, advised that the Building Department has received an <br />application for a house move as submitted by Lykowski <br />Construction, Inc., 21707 West Ireland Road, South Bend, Indiana. <br />Lykowski Construction intends to move a house from the northwest <br />corner of Corby and Hickory to a parcel west of 16860 Catalpa. <br />Lykowski Construction has provided the releases from the utility <br />companies and clearance has been given by the Police Department <br />and the Division of Engineering. The St. Joseph County Board of <br />Commissioners has also given their approval for the move and all <br />zoning requirements are being met with the new location. It is <br />noted that the Board is also in receipt of the required <br />Certificate of Insurance naming the City of South Bend as an <br />additional insured. Mr. Fozo requested the Board's approval of <br />this request. Therefore, Mrs. Mueller made a motion that this <br />request be approved. Mr. Caldwell seconded the motion which <br />carried. <br />FAVORABLE RECOMMENDATION - VACATION OF THE WEST 116 FEET OF THE <br />1ST E/W ALLEY N. OF LA SALLE STREET RUNNING BETWEEN NOTRE DAME <br />AVENUE AND FRANCES STREET <br />Mr. Leszczynski indicated that the Common Council has submitted to <br />the Board a Vacation Petition as signed by Mr. William T. <br />Hambling, Director of Project Management, Saint Joseph's Medical <br />Center, Inc., 801 East LaSalle Street, P.O. Box 1953, South Bend, <br />Indiana, and listing Attorney John B. Ford, 1800 Valley American <br />
