REGULAR MEETING JANUARY 11, 1993 <br />009 <br />of the Board that submitted for consideration was a Health <br />Options Plan Agreement as submitted by Home Health Services, <br />Inc., (HHS, Inc.), 5363. 44th Street, S.E., Grand Rapids, <br />Michigan. Services that HHS, Inc., will provide to the City <br />include hospital pre -admission review, concurrent review and home <br />treatment coordination. Mrs. Manier noted that this service is <br />intended, to keep health insurance costs down. Mr. Leszczynski <br />made a motion that the Agreement be approved. Mr. Caldwell <br />seconded the motion which carried. <br />ADOPT RESOLUTION NO. 1-1993 DISPOSAL OF OBSOLETE VEHICLE <br />In a letter to the Board, Mr. Matthew L. Chlebowski, Interim <br />Director, Division of Equipment Services,'advised.that the <br />Division requests that the Board declare obsolete a 1981 single <br />axle dump truck that has extensive cracks in the frame as well as <br />major engine problems. To repair this unit would cost seventy <br />five per cent (75%) of a new unit which is excessive. The <br />Division of Equipment Services will salvage any and all parts and <br />components for future use on a restoration. The cab and frame <br />will be sent to the salvage yard. <br />Therefore, Mr. Leszczynski made a motion that the request be <br />approved and the appropriate Resolution which has been prepared <br />be adopted. Mr. Caldwell seconded the motion which carried and <br />the following Resolution No. 1-1993 was adopted: <br />RESOLUTION NO. 1-1993 <br />A RESOLUTION OF THE SOUTH BEND BOARD OF PUBLIC WORKS <br />ON DISPOSAL OF UNFIT AND/OR OBSOLETE PROPERTY <br />WHEREAS, it has been determined by the Board of Public Works <br />that the following property is unfit for the purpose for which it <br />was intended and is no longer needed by the City of South Bend: <br />1981 SINGLE AXLE DUMP TRUCK <br />VIN. # 1M2N112B4A002883 <br />WHEREAS, Indiana Code 36-1-11-6(c) permits and establishes <br />procedure for disposal of personal property which is unfit for <br />the purpose for which it was intended and which is no longer <br />needed by the City. <br />NOW, THEREFORE, BE IT RESOLVED by the Board of Public Works <br />of the City of South Bend that the one (1) item listed above is <br />no longer needed by the City, is unfit for the purpose for which <br />it was intended; and has an estimated value of less than One <br />Thousand Dollars ($1,000.00). <br />BE IT FURTHER RESOLVED, that said equipment may be <br />transferred or sold at public auction or private sale, without <br />advertising.
