REGULAR MEETING OCTOBER 31, 1994 <br />APPROVE CHANGE ORDER - COLLEGE FOOTBALL HALL OF FAME - COUP ELECTRIC <br />Mr. Leszczynski advised that Mr. Rick Dieringer, Verkler, Inc., 4406 Technology Drive, South <br />Bend, Indiana, has submitted Change Order No. 16.00-04 indicating that the Contract amount be <br />increased $43,000.00 for a new Contract sum including this Change Order in the amount of <br />$492,836.00. Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the <br />Change Order was approved. <br />APPROVE CONSTRUCTION CONTRACT - UN KOTE SANITARY SEWER EXTENSION - <br />LIFT STATIONS NO. 2.3 AND 4 - SLUTSKY-PELTZ PLUMBING & HEATING - PROJECT <br />NO.94-72 <br />Mr. Leszczynski advised that in accordance with the bid awarded on October 10, 1994, to Slutsky- <br />Peltz Plumbing & Heating, 844 S. Rush Street, South Bend, Indiana, in the amount of $47,000.00 <br />for the above referred to project, a Contract in said amount was being submitted for Board approval. <br />Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the Contract was <br />approved and the appropriate Certificate of Insurance, Performance Bond and Labor and Materials <br />Payment Bond as submitted were filed. <br />APPROVE COMMUNITY DEVELOPMENT CONTRACT <br />The following Community Development Contract was presented to the Board for approval: <br />- 1994-1995 Housing Development Corporation of St. Joseph County Contract for Landlord Loan <br />Program with Section 108 Loan funds in the amount of $300,000.00 Upon a motion made by Mr. <br />Caldwell, seconded by M. Leszczynski and carried, the above referred to Contract was approved and <br />executed. <br />APPROVAL OF THE NAMING OF A DEDICATED ALLEY - FARRELL LANE <br />Mr. Leszczynski advised the Board is in receipt of favorable recommendations from the Division <br />of Engineering, Police Department, Fire Department, Street Department and Department of <br />Sanitation that the existing alley which enters lot "J" of Witchoe and Keller subdivision be named <br />Farrell Lane. Mr. and Mrs. Wayne Miller, 1400 Farrell Lane, South Bend, Indiana, have purchased <br />the lot and have their home on it. Additionally, they have paid to have the alley paved and agree to <br />provide their own maintenance and snow removal. Therefore, upon a motion made by Mrs. Manier, <br />seconded by Mr. Caldwell and carried, the naming of Farrell Lane was approved. <br />AWARD BID - SOUTHEAST REDEVELOPMENT AREA SITE CLEARANCE PHASE I <br />STRUCTURE REMOVAL PACKAGE 2 - B & J EXCAVATION - PROJECT NO 94-72 <br />Mr. Carl P. Littrell, Director, Division of Engineering, advised the Board that on October 24, 1994, <br />bids were received and opened for the above referred to project. After reviewing those bids, Mr. <br />Littrell recommends that the Board award the contract to B & J Excavation, 1104 W. Donald Street, <br />South Bend, Indiana the amount of $89,975.00. Therefore, Mr. Caldwell made a motion that the <br />recommendation be accepted and the bid be awarded as outlined above. Mrs. Manier seconded the <br />motion which carried. <br />APPROVE CHANGE ORDER NO 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - <br />BLACKTHORN GOLF COURSE - PROJECT NO 92-41 <br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manager, Division of Engineering, <br />has submitted Change Order No. 1 (Final) indicating that the contract amount be increased by <br />$126,180.73 for a new contract sum including this Change Order in the amount of $2,379,215.03. <br />Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon <br />a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, Change Order No. 1 <br />(Final) and the Project Completion Affidavit were approved subject to the filing of the appropriate <br />three-year Maintenance Bond. <br />REQUEST TO ENCROACH ONTO PUBLIC -RIGHT -OF WAY - BENCHWARMERS SPORTS <br />BAR - 236 SOUTH MAIN STREET <br />Mr. Leszczynski advised the Board that City Awning of South Bend, 1731 S. Franklin, South Bend, <br />Indiana, is requesting permission to install three (3) triangular style fabric awnings at Benchwarmers <br />Sports Bar, 236 South Main Street, South Bend, Indiana. Further, the awnings will install onto the <br />building and extend outward over the City sidewalk. Upon a motion made by Mr. Leszczynski, <br />
