<br />Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the above bids were
<br />referred to the Fire Department for review and recommendation.
<br />CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT -
<br />BLACKTHORN GOLF COURSE
<br />Mr. Caldwell advised Board members that the Project Completion Affidavit has been submitted for
<br />the above referred to project. However, Change Order No. 1 (Final), for Salyers Golf Construction,
<br />Inc., has not been returned at this time. Upon a motion made by Mrs. Manier, seconded by Mr.
<br />Caldwell and carried, Change Order No. I (Final) and the Project Completion Affidavit were tabled
<br />subject to the submittal of the Final Change Order.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT - MICHIGAN STREET LIFT STATION
<br />VALVES REPLACEMENT - PROJECT NO. 94-12
<br />Mr. Caldwell advised that Mr. Roberto Flores, Division of Environmental Services, has submitted
<br />a Project Completion Affidavit for the above referred to project, from Dye Plumbing & Heating,
<br />3535 Monroe Street, LaPorte, Indiana, with the final cost of $17,042.00. Upon a motion made by
<br />Mr. Caldwell, seconded by Mrs. Manier and carried, the Project Completion Affidavit was approved
<br />subject to the filing of the appropriate three-year Maintenance Bond.
<br />APPROVE CHANGE ORDER - COLLEGE FOOTBALL HALL OF FAME - HAGERMAN
<br />CONSTRUCTION CORPORATION
<br />Mr. Caldwell advised that Mr. R.A. Dieringer, Verkler, Inc., 4406 Technology Drive, South Bend,
<br />Indiana, has submitted Change Order No. 03.00-01, on behalf of Hagerman Construction
<br />Corporation, Post Office Box 10690, Ft. Wayne, Indiana, indicating that the Contract amount be
<br />increased $621.00 for a new Contract sum including this Change Order in the amount of
<br />$592,621.00. Upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the
<br />Change Order was approved.
<br />APPROVE CONSTRUCTION CONTRACT - CLEVELAND ROAD IMPROVEMENTS -
<br />PORTAGE TO BENDIX - PROJECT NO.94-48
<br />Mr. Caldwell advised that in accordance with the bid awarded on September 12, 1994, to Walsh &
<br />Kelly, 24358 State Road 223, South Bend, Indiana, in the amount of $125,466.40 for the above
<br />referred to project, a Contract in said amount was being submitted for Board approval. Upon a
<br />motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the Contract was approved and
<br />the appropriate Certificate of Insurance, Performance Bond and Labor and Materials Payment Bond
<br />as submitted were filed.

<br />RENOVATIONS - PROJECT NO. 94-52
<br />Mr. Caldwell advised that in accordance with the bid awarded on September 26, 1994, to Maddox
<br />Industrial Contractors, Inc., 3060 West Minnesota Street, Indianapolis, Indiana, in the amount of
<br />$2,422,000.00 for the above referred to project, a Contract in said amount was being submitted for
<br />Board approval. Upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the
<br />Contract was approved and the appropriate Certificate of Insurance, Performance Bond and Labor
<br />and Materials Payment Bond as submitted were filed.
