�,-1 ", 913 REGULAR MEETING CHANGE ORDER NO. 16.00-03 CONTRACTOR: ORIGINAL CONTRACT SUM: DECREASE: NEW CONTRACT AMOUNT: SEPTEMBER 26, 1994 Colip Electric Company P.O. Box 840 South Bend, Indiana $449,750.00 $ 1,313.00 $449,836.00 Mr. Caldwell made a motion that the above referred to Change Orders be approved. Mrs. Manier seconded the motion which carried. AWARD BID - EDISON WATER TREATMENT PLANT RENOVATIONS Mr. Caldwell advised that it has been recommended that the Board award the bid of Maddox Industrial Contractors, Inc., 3060 West Minnesota Street, Indianapolis, Indiana, in the amount of $2,422,000.00 for the Edison Filtration Plant Renovations. It is noted that bids for this renovation were opened by the Board on September 12, 1994. Mrs. Manier made a motion that the recommendation be accepted and the bid be awarded. Mr. Caldwell seconded the motion which carried. AWARD BID - F. JAY NIMTZ PARKWAY - PHASE III Mr. Caldwell advised that it has been recommended that the Board award the bid of Rieth-Riley Construction, Post Office Box 1775, South Bend, Indiana, in the amount of $638,631.15 for the above referred to project. It is noted that bids for this project were opened by the Board on September 6, 1994. Mrs. Manier made a motion that the recommendation be accepted and the bid be awarded. Mr. Caldwell seconded the motion which carried. APPROVAL OF FINAL INSPECTION REPORT - EDISON ROAD Mr. Caldwell informed members that a Report of Contract Final Inspection and Recommendation for Acceptance from the Indiana Department of Transportation for the Edison Road Project, Contract R-20304, MASTP-STP-S004(4) has been submitted for approval. Mr. Caldwell made a motion that the report be accepted and approved. Mrs. Manier seconded the motion which carried. APPROVE PROJECT COMPLETION AFFIDAVIT - STUDEBAKER LOT #10 1424 SOUTH LAFAYETTE - UNDERGROUND STORAGE TANK REMOVAL - PROJECT NO. 94-44 Mr. Caldwell advised that Warner & Sons, Inc., 29099 U.S. Highway 33, Elkhart, Indiana, has submitted a Project Completion Affidavit for the above referred to project, making the final Contract amount $3,220.00. Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the Project Completion Affidavit was approved. REQUEST FOR LICENSE APPLICATION - SECOND HAND STORE Mr. Caldwell advised that the Board is in receipt of a License Application for Secondhand Store from Robert R. Harrison, d/b/a Harrison Furniture and Treasures, 1609 S. Miami, South Bend, Indiana. Upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the above referred to application was forwarded to the appropriate Bureaus for recommendation. REQUEST FOR TRANSIENT MERCHANTS LICENSE Mr. Caldwell advised that the Board is in receipt of a License Application for Transient Merchants from Sharon H. Drybrough on behalf of Tranex Car Sales, Inc., d/b/a/ Va1Car Rental Car Sales, 5075 W. 38th Street, Indianapolis, Indiana. Upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the above referred to application was forwarded to the appropriated Bureaus for recommendation. RECOMMENDATION TO VACATE AN ALLEY Mr. Caldwell stated that the Board is in receipt of a request to vacate the following alley: The first north -south alley east of South Michigan Street from the south right-of-way line of East Broadway Street to the north right-of-way line of the first intersecting east -west alley south of East Broadway Street for a distance of approximately 153 feet and a width of approximately 14 feet. Part situated in Haney's Addition to the City of South Bend, Indiana.
