PROPOSAL: All accounts will be collected at a rate of thirty per cent (30%). FEDERATED COLLECTION AGENCY 2015 West Western Avenue, Suite 138 South Bend, Indiana 46629 Proposal was submitted by Mr. Robert Forgash, President. PROPOSAL: Federated Collection Agency shall retain thirty per cent (30%) and if legal action needs to be taken fifty per cent (50%) for all monies collected. No fees will be accessed on unrecovered accounts. CREDIT BUREAU OF SOUTH BEND-MISHAWAKA 312 West Colfax Avenue South Bend, Indiana 46634 Proposal was submitted by Mr. Steve C. VanderMolen, Bureau Manaqer. POSAL: Fee Program is on a contingency basis. No collection, no fee. The Credit Bureau will collect the City's accounts at twenty-nine per cent (29%).
