<br />Road from Ireland Road north to Randolph Street, as submitted by
<br />Cole Associates, Inc., 2211 East Jefferson Boulevard, South Bend,
<br />Indiana. Mr. Caldwell noted that this Supplemental Agreement
<br />amends the Agreement executed on January 2, 1990. The change in
<br />contract price is $84,860.50 with the total fee not to exceed
<br />$409,224.00.
<br />Additionally submitted was Supplemental Agreement No. 2 for this
<br />same project indicating additional work to be completed by Cole
<br />Associates. The total change in contract price as contained in
<br />this Agreement is $169,776.00 with the total fee not to exceed
<br />$579,000.00.
<br />Mr. Carl P. Littrell, Director, Division of Engineering, noted
<br />that Supplemental Agreement No. 1 is for design of lift stations.
<br />Supplemental Agreement No. 2 is for redesigning of the road to
<br />have less of an adverse effect on the properties owners on the
<br />west side of Ironwood Road.
<br />Mr. Caldwell made a motion that Supplemental Agreement No. 1 and
<br />Supplemental Agreement No. 2 be approved. Mrs. Manier seconded
<br />the motion which carried.
<br />APPROVE SUPPLEMENTAL AGREEMENT #4 - TWYCKENHAM DRIVE BETWEEN MC
<br />KINLEY STREET AND SOUTH BEND AVENUE
<br />The Board is in receipt of Supplemental Agreement No. 4 - Revised,
<br />between the City and Cole Associates, Inc., 2211 East Jefferson
<br />Boulevard, South Bend, Indiana. This Agreement indicates
<br />additional services for revisions to construction documents to
<br />extend the northerly construction limits to State Road #23, modify
<br />the signal design for two lane traffic at the State Road
<br />23/Twyckenham intersection, right-of-way management and
<br />acquisition services for twenty-four (24) temporary right -of- way
<br />parcels. It is noted that the original Agreement was approved by
<br />the Board on August 5, 1985. Further, this Agreement increases
<br />the contract fee of $277,920.00 by $20,240.00 for a new total of
<br />$298,160.00.
<br />Mr. Carl P. Littrell, Director, Division of Engineering, advised
<br />that this Agreement adds the intersection of South Bend Avenue and
<br />Twyckenham to the project. Mr. Caldwell made a motion that this
<br />Supplemental Agreement #4 be approved and executed. Mrs. Manier
<br />seconded the motion which carried.
<br />APPROVE CONSTRUCTION CONTRACTS:
<br />RENOVATION/INTERIOR FINISHES OF CURRENT OFFICE SPACE AT 113
<br />WEST COLFAX AVENUE
<br />STEPHENSON MILLS APARTMENTS SITE DEVELOPMENT
<br />Mr. Caldwell advised that in accordance with the Quotation awarded
<br />on February 14, 1994, to Ziolkowski Construction, Inc., 1005 South
<br />Lafayette Boulevard, P.O. Box 1106, South Bend, Indiana, in the
<br />amount of $11,330.00, for the above referred to project, a
<br />Contract in said amount was being submitted for Board approval.
<br />Additionally, in accordance with the Quotation awarded on February
<br />21, 1994, to Warner & Sons, Inc., P.O. Box 87, Elkhart, Indiana,
<br />in the amount of $7,765.00 for the Stephenson Mills Apartments
<br />Site Development project, the appropriate Contract was submitted
<br />for consideration.
<br />Upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and
<br />carried, the above referenced Contracts were approved.
