0617 <br />REGULAR MEETING <br />JANUARY 31, 1994 <br />approved and executed. Mr. Leszczynski seconded the motion which <br />carried. <br />APPROVE HEALTH OPTIONS PLAN AGREEMENT AND MENTAL HEALTH OPTIONS <br />PLAN AGREEMENT - HOME HEALTH SERVICES, INC. <br />Board Attorney Mary H. Mueller indicated that these Agreements <br />submitted today are pre -certification agreements. <br />The first Agreement between the City and Home Health Services, <br />Inc., 5363 44th Street, SE, Grand Rapids, Michigan, will provide a <br />program which comprises the services of hospital pre -admission <br />review, concurrent review and home treatment coordination. The <br />initial term of this Agreement shall be for one (1) year <br />commencing January 1, 1994 and ending December 31, 1994. This <br />Agreement shall automatically be renewed for successive one (1) <br />year terms commencing each January 1, unless either party gives <br />the other written notice to terminate. However, this Agreement <br />shall be subject to annual appropriation by the City. <br />The Mental Health Options Plan Agreement between the City and Home <br />Health Services, Inc., indicates that HHS will provide services <br />which include mental health facility pre -admission certification, <br />outpatient certification, concurrent review and intensive <br />outpatient therapy coordination and monitoring. The term of this <br />Agreement is one (1) year commencing January 1, 1994 and ending <br />December 31, 1994. This Agreement shall automatically be renewed <br />for successive one (1) year terms commencing each January 1, <br />unless either party gives the other written notice to terminate. <br />However, this Agreement shall be subject to annual appropriation <br />by the City. <br />Mr. Caldwell made a motion that these Agreements be approved. Mr. <br />Leszczynski seconded the motion which carried. <br />APPROVE CONTRACT FOR CONSULTING SERVICES - MTC SERVICES <br />Mr. Leszczynski advised that the Board is in receipt of a Contract <br />for Consulting Services between MTC Services and the City, by and <br />through the Board of Public Works on behalf of the Department of <br />Economic Development. MTC Services agrees to perform professional <br />assistance in the overall operation of the neighborhood business <br />assistance programs. The term of this Agreement shall be from <br />January 1, 1994 to and including December 31, 1994. Additionally, <br />MTC Services shall be compensated the amount of $26,952.00 for <br />these services. Mr. Leszczynski made a motion that this Contract <br />be approved and executed. Mr. Caldwell seconded the motion which <br />carried. <br />APPROVE PROPOSAL - STRATEGIC INFORMATION SYSTEMS - DIVISION OF <br />ENVIRONMENTAL SERVICES <br />Mr. Jack Dillon, Director, Division of Environmental Services, <br />submitted to the Board a Strategic Information Systems Proposal <br />for the Division of Environmental Services as received from Thomas <br />Beug & Associates, 2592 North Summit Avenue, Milwaukee, Wisconsin. <br />Mr. Leszczynski noted that this Proposal relates to the computer <br />system at the Division of Environmental Services. It is further <br />noted that this Proposal is for a not to exceed cost of <br />$24,000.00. Mr. Leszczynski made a motion that this Proposal be <br />approved. Mr. Caldwell seconded the motion which carried. <br />ADOPT RESOLUTION NOS. 4-1994 AND 5-1994 - DISPOSAL OF SERVICE <br />REVOLVERS <br />In a letter to the Board, Police Chief Ronald G.
