068 <br />REGULAR MEETING <br />JANUARY 31, 1994 <br />The regular meeting of the Board of Public Works was convened at <br />9:35 a.m. on Monday, January 31, 1994, by Board President John E. <br />Leszczynski, with Mr. Leszczynski, Mr. James R. Caldwell and Mrs. <br />Jenny Pitts Manier present. Also present was Assistant City <br />Attorney Mary H. Mueller. <br />APPROVE MINUTES OF PREVIOUS MEETING <br />Upon a motion made by Mr. Caldwell, seconded by Mr. Leszczynski <br />and carried, the minutes of the regular meeting of the Board held <br />on January 24, 1994, were approved. <br />APPROVE COMMUNITY DEVELOPMENT CONTRACT AND ADDENDA <br />The following Community Development Contract and Addenda were <br />presented to the Board for approval: <br />CONTRACT <br />COMMUNITY HOMEBUYERS CORPORATION. <br />HOUSING DEVELOPMENT FUND - CHC ACQUISITION/REHAB PROGRAM <br />The targeted goal for the activities under the CHC <br />Acquisition/Rehab Program (CD90-108, CD91-108, CD92-108(B)) <br />is to assist twenty (20) low and moderate income homeowners <br />acquire or acquire/rehab their primary dwelling. The total <br />amount of this Contract shall not exceed $650,000.00. Funds <br />for this program include $250,000.00 from a 1990 entitlement <br />grant, $105,000.00 from a 1991 grant and a portion, <br />$295,000.00 of a 1992 grant. <br />Mr. Caldwell made a motion that this Contract be approved and <br />executed. Mr. Leszczynski seconded the motion which carried. <br />Additionally, the following Community Development Addenda were <br />presented for consideration: <br />ADDENDUM III <br />DIVISION OF PLANNING AND NEIGHBORHOOD DEVELOPMENT <br />NEIGHBORHOOD PARTNERSHIPS PROGRAM <br />Addendum III to the 1993 Partnership Contract extends the <br />Contract to December 31, 1994. Also, this Addenda adds the <br />eligible activity of site improvements to support the <br />development of low and moderate income housing. <br />ADDENDUM III <br />DIVISION OF PLANNING AND NEIGHBORHOOD DEVELOPMENT <br />WEST WASHINGTON PUBLIC WORKS ACTIVITY <br />Addendum III extends the Contract to May 31, 1994. <br />ADDENDUM I <br />NEAR NORTHWEST NEIGHBORHOOD INC. <br />NEAR NORTHWEST NEIGHBORHOOD ASSISTANCE ACTIVITY <br />Addendum I extends the Contract to February 28, 1994. <br />Upon a motion made by Mr. Caldwell, seconded by Mr. Leszczynski <br />and carried, the above referred to Addenda were approved and <br />executed. <br />APPROVE MENTAL HEALTH PROVIDER AGREEMENT - MADISON CENTER. INC. <br />Board Attorney Mary H. Mueller informed members of the Board that <br />the Agreement submitted today is a two (2) year Agreement with <br />Madison Center, Inc., 403 East Madison, South Bend, Indiana. <br />Through this Agreement, Madison Center provides to the City a <br />fifteen per cent (150) reduction in the amount of its schedule of <br />fees for any covered services furnished to an employee of this <br />City.
