REGULAR MEETING DECEMBER I I. 1995 <br />Bid was signed by Mr. Jack Warner, President <br />Non -Collusion Affidavit was in order <br />Non -Discrimination Commitment form was completed <br />Five per cent (5%) Bid Bond was submitted <br />BID: $310,599.75 <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Caldwell and carried, the above bids <br />were referred to the Division of Engineering for review and recommendation. <br />AGREEMENTS TABLED - NEIGHBORHOOD RESOURCES AND TECHNICAL SERVICES <br />CORPORATION <br />Upon a motion made by Mr. Leszczynski, seconded by Mr. Caldwell and carried, the above referred <br />to Agreements were tabled until the December 18,1995 meeting, as Mr. Caldwell is a Neighborhood <br />Resources and Technical Services Corporation Board member, and a lack of quorum existed. <br />APPROVE CHANGE ORDER NO. 1 and 2 (FINAL) AND PROJECT COMPLETION <br />AFFIDAVIT - STUDEBAKER NATIONAL MUSEUM - SPRINKLER SYSTEM - PROJECT NO. <br />95-48 <br />Mr. Leszczynski advised that Mr Phil Cook, Shambaugh & Sons, Inc., 7614 Opportunity Drive, Fort <br />Wayne, Indiana, has submitted Change Orders No. 1 and 2 (Final) indicating that the contract <br />amount be increased by $2,400.00 (Change Order 1) and $511.00 (Change Order 2) for a new <br />contract sum including these Change Orders in the amount of $27,237.00. Additionally submitted <br />was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. <br />Leszczynski, seconded by Mr. Caldwell and carried, Change Orders No. 1 and 2 (Final) and the <br />Project Completion Affidavit were approved subject to the filing of the appropriate three-year <br />Maintenance Bond. <br />APPROVAL OF RECOMMENDATION - RIVERSIDE RUN/WALK ROADRACE <br />Mr. Leszczynski stated that favorable recommendations have been received by the Division of <br />Engineering, Park Department, Police Department and the City Attorney's office, in reference to a <br />request as submitted by Ms. Susan O'Connor, South Bend Recreation Department to conduct the <br />above referred to run/walk roadrace, on Saturday, June 29, 1996, beginning at 7:00 a.m., on the <br />designated route as submitted. Therefore, upon a motion made by Mr. Leszczynski, seconded by <br />Mr. Caldwell and carried, the recommendation was approved. <br />APPROVAL OF RECOMMENDATION - ERSKINE BOULEVARD CANDLELIGHT WALK <br />Mr. Leszczynski stated that favorable recommendations have been received by the Division of <br />Engineering, Park Department, Police Department and the City Attorney's office, in reference to a <br />request as submitted by Mr. Richard Cowper, Erskine Boulevard Association, 2720 Erskine <br />Boulevard, South Bend, Indiana, to conduct the above referred to Candlelight Walk, on Saturday, <br />December 16,1995, from 6:00 p.m. until 9:00 p.m., on the designated route as submitted. Therefore, <br />upon a motion made by Mr. Leszczynski, seconded by Mr. Caldwell and carried, the <br />recommendation was approved. <br />FAVORABLE RECOMMENDATION - PETITION TO VACATE A STREET <br />Mr. Leszczynski indicated that the Common Council has submitted to the Board a Vacation Petition <br />as completed by Mr. Thomas E. Squibb, President, Milton Inc., Post Office Box 5027, Mishawaka, <br />Indiana to vacate the following street: <br />East Madison Street from the east line of the first north south alley east of North Michigan <br />Street to the west line of Saint Joseph River plat situated in the original plat of South Bend, <br />St. Joseph County, Indiana <br />Mr. Leszczynski advised that the Board is in receipt of favorable recommendations concerning this <br />Vacation Petition from the Area Plan Commission, Police Department, Fire Department, Sanitation, <br />Department of Economic Development and the Department of Public Works. Therefore, Mr. <br />Caldwell made a motion that the Board send a favorable recommendation to the Common Council <br />concerning this Vacation Petition. Mr. Leszczynski seconded the motion which carried. <br />
