REGULAR MEETING OCTOBER 23, 1995
<br />APPROVE CHANGE ORDER - CENTURY CENTER REMODELING - PROJECT NO. 94-83
<br />Mr. Leszczynski advised that Mr. Joseph Mathews, Mathews-Purucker-Anella, Inc., 218 South
<br />Frances, South Bend, Indiana, has submitted Change Order No. G-13, on behalf of Reinke
<br />Construction, Post Office Box 3095, South Bend, Indiana indicating that the Contract amount be
<br />increased $7,360.00 for a new Contract sum including this Change Order in the amount of
<br />$2,380,219.55. Upon a motion made by Mrs. Manier, seconded by Mr. Leszczynski and carried,
<br />the Change Order was approved.
<br />APPROVE PROJECT COMPLETION AFFIDAVIT - FIELD COATING - IRELAND STATION
<br />RESERVOIR - PROJECT NO. 94-94
<br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manager, has submitted the
<br />Project Completion Affidavit on behalf of Icarus Industrial Painting and Contracting Co., Inc., Lake
<br />Station, Indiana, for the above referred to project, indicating a final cost of $240,932.50. Upon a
<br />motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the Project Completion
<br />Affidavit was approved.
<br />APPROVE CONSENT TO ANNEXATION AND WAIVER OF RIGHTS TO REMONSTRATE
<br />Mr. Leszczynski stated that the Board is in receipt of a Consent to Annexation and Waiver of Right
<br />to Remonstrate as submitted by Ms. Ruth Yarger. The Consent indicates that in consideration for
<br />permission to provide water service to 17180 McErlain, South Bend, Indiana, Ms. Yarger waives
<br />and releases any and all rights to remonstrate against or oppose any pending or future annexation
<br />of the property by the City of South Bend. Therefore, Mr. Leszczynski made a motion that the
<br />Consent be approved. Mrs. Manier seconded the motion which carried.
<br />APPROVAL OF RECOMMENDATION - TO CONDUCT ANNUAL PARK AVENUE
<br />ASSOCIATION HALLOWEEN PARTY
<br />Mr. Leszczynski stated that favorable recommendations have been received by the Division of
<br />Engineering, Police Department and the City Attorney's office, in reference to a request as
<br />submitted by Mr. Stanley Molenda, President, Park Avenue Neighborhood Association, 329 West
<br />Madison, South Bend, Indiana to conduct the above referred to Halloween party, on Sunday,
<br />October 29, 1995, from 3:00 p.m. until 7:00 p.m., on Manitou Place from Park Avenue to North
<br />Forest Avenue. Upon a motion made by Mrs. Manier, seconded by Mr. Leszczynski and carried,
<br />the recommendation was approved.
<br />APPROVAL OF RECOMMENDATION - TO CONDUCT UNITED RELIGIOUS COMMUNITY
<br />CROP WALK
<br />Mr. Leszczynski stated that favorable recommendations have been received by the Division of
<br />Engineering, Park Department, Police Department and the City Attorney's office, in reference to
<br />a request as submitted by Mr. James Fisko, Executive Director, United Religious Community, 2015
<br />Western Avenue, South Bend, Indiana to conduct the above referred to CROP Walk, on Sunday,
<br />September 29, 1996, from 2:00 p.m. until 5:00 p.m., on the designated route as submitted. Upon
<br />a motion made by Mrs. Manier, seconded by Mr. Leszczynski and carried, the recommendation was
<br />approved.
<br />APPROVAL OF RECOMMENDATION - TO CONDUCT RED RIBBON MARCH
<br />Mr. Leszczynski stated that favorable recommendations have been received by the Division of
<br />Engineering, Park Department, Police Department and the City Attorney's office, in reference to
<br />a request as submitted by Ms. Gladys Muhammad, United Community Against Alcohol Drug
<br />Abuse, 914 Lincolnway West, South Bend, Indiana to conduct the above referred to march, on
<br />Friday, October 27, 1995, from 10:00 a.m. until 1:00 p.m., on the designated route as submitted.
<br />Upon a motion made by Mrs. Manier, seconded by Mr. Leszczynski and carried, the
<br />recommendation was approved.
