<br />APPROVAL OF AGREEMENT - SALE OF RETIRED POLICE K-9 (MAX)
<br />Mr. Leszczynski stated that the Board is in receipt of an Agreement between the City of South Bend
<br />and Mr. Gary Jerzak, South Bend, Indiana, for the sale of a retired Police Department K-9 dog,
<br />"Max". Mr. Leszczynski noted that the Agreement is in the amount of $1.00, and the K-9 dog was
<br />declared obsolete by Resolution No. 36-1996, at the May 6, 1996 meeting. Therefore, upon a
<br />motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the Agreement was approved
<br />and executed.
<br />APPROVAL OF SUPPLEMENTAL AGREEMENTS - TEAMSTERS LOCAL UNION NO. 364
<br />AGREEMENT
<br />Mr. Leszczynski stated that the Board is in receipt of nine (9) Supplemental Agreements between
<br />the City of South Bend and the Teamsters Local Union No. 364, as submitted:
<br />Division of Equipment Services Bureau of Sewers
<br />Bureau of Organic Resources Street Department, Maintenance Division
<br />Police Department Bureau of Streets
<br />Bureau of Solid Waste Bureau of Traffic & Lighting
<br />Bureau of Wastewater
<br />Therefore, upon a motion made by Mrs. Manier, seconded by Mr. Caldwell and carried, the
<br />Supplemental Agreements were approved and executed.
<br />APPROVE CHANGE ORDERS NO. 1 AND 2 (FINAL) AND PROJECT COMPLETION
<br />AFFIDAVIT - COLLEGE FOOTBALL HALL OF FAME - BUILDING SIGNAGE
<br />Mr. Leszczynski advised that Mr. Ralph Rhodes, Hilferty & Associates, 14240 Route 550, Athens,
<br />Ohio has submitted the following Change Orders on behalf of Exhibit Concepts, 700 Crossroads
<br />Court, Vandalia, Ohio, and recommended approval:
<br />Change Order No. 1 Decrease $11,147.00 $165,528.00
<br />Change Order No. 2 Increase $13,720.00 $179,248.00
<br />Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon
<br />a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the Change Orders (Final)
<br />and the Project Completion Affidavit were approved.
