Or," <br />REGULAR MEETING MAY 6, 1996 <br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS SOUTHEAST <br />PARTNERSHIP CENTER SERVICE AREA - SIDEWALK CURB AND APPROACH REPAIRS - <br />PROJECT NO. 96-35 <br />In a memorandum to the Board, Mr. Dan Weisel, Division of Engineering, requested permission to <br />advertise for the receipt of bids for the above referred to project. Mr. Weisel stated that this project <br />will repair concrete sidewalks, curbs, driveway and alley approaches and other appurtenant work <br />in the above referenced area. Additionally, the source of funding for this project will be Community <br />Development Block Grant. Therefore, upon a motion made by Mr. Caldwell, seconded by Mr. <br />Leszczynski and carried, the request to advertise for the receipt of bids was approved. <br />APPROVE APPLICATION FOR LONG TERM DISABILITY BENEFITS - THE GIBSON <br />INSURANCE AGENCY <br />Mr. Leszczynski stated that an Application for Long Term Disability Benefits was sumitted to the <br />Board for approval by The Gibson Insurance Agency, Indianapolis, Indiana. Mr. Leszczynski stated <br />that this Application is for the City of South Bend employee disability benefits, excluding Fire and <br />Police. Therefore, upon a motion made by Mr. Caldwell, seconded by Mr. Leszczynksi and carried, <br />the Application for Long Term Disability Benefits was approved. <br />ADOPT RESOLUTION NO. 36-1996 - DISPOSAL OF CITY OWNED PROPERTY <br />Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the following <br />Resolution was adopted by the Board of Public Works: <br />RESOLUTION NO. 36-1996 <br />A RESOLUTION OF THE SOUTH BEND <br />BOARD OF PUBLIC WORKS <br />REGARDING THE RETIREMENT OF <br />"MAX". A K-9 POLICE DOG <br />WHEREAS, the South Bend Board of Public Works has been advised by the City's Police <br />Chief that "Max", a K-9, is no longer needed by the South Bend Police Department; and <br />WHEREAS, it has been determined by the Board of Public Works that "Max" is unfit for the <br />purpose for which he was acquired and is no longer of use to the City of South Bend; and <br />WHEREAS, Gary Jerzak, South Bend, Indiana, has agreed to care for "Max" at his own <br />expense and to keep him as a house pet and understands that "Max" can never again be used as a <br />Police Dog; and <br />WHEREAS, I.C. 36-1-11-6 © permits and establishes a procedure for disposal of property <br />which is unfit for the purpose for which it was intended and which is no longer needed by the City. <br />NOW, THEREFORE, BE IT RESOLVED, by the Board of Public Works of the City of <br />South Bend, Indiana, that "Max", a K-9 Police Dog, is unfit for the purpose for which he was <br />acquired, is no longer of use to the City of South Bend, and has an estimated value of less than One <br />Thousand Dollars ($1,000.00). <br />BE IT FURTHER RESOLVED, that "Max" the Police Dog, should be retired and put into <br />the care of Gary Jerzak for use as a house pet only. <br />ADOPTED this 6th day of May, 1996. <br />CITY OF SOUTH BEND <br />BOARD OF PUBLIC WORKS <br />s/John Leszczynski, President <br />
