REGULAR MEETING <br />APRIL 29, 1996 <br />APPROVE PROJECT COMPLETION AFFIDAVIT - COLLEGE FOOTBALL HALL OF FAME - <br />BUILDING CONSTRUCTION - PROJECT NO. 93-09 <br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manager, has submitted the <br />Project Completion Affidavit on behalf of Hagerman Construction, Post Office Box 10690, Fort <br />Wayne, Indiana, for the above referred to project, indicating a final cost of $587,375.63. Upon a <br />motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the Project Completion <br />Affidavit was approved. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - COLLEGE FOOTBALL HALL OF FAME - <br />TUNNEL CONSTRUCTION - PROJECT NO.93-09 <br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manager, has submitted the <br />Project Completion Affidavit on behalf of Hagerman Construction, Post Office Box 10690, Fort <br />Wayne, Indiana, for the above referred to project, indicating a final cost of $348,527.00. Upon a <br />motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the Project Completion <br />Affidavit was approved. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - COLLEGE FOOTBALL HALL OF FAME - <br />SITE CONCRETE - PROJECT NO.93-09 <br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manager, has submitted the <br />Project Completion Affidavit on behalf of Casteel Construction, 23186 West Ireland Road, South <br />Bend, Indiana, for the above referred to project, indicating a final cost of $170,459.00. Upon a <br />motion made by Mr. Caldwell, seconded by Mr. Leszczynski and carried, the Project Completion <br />Affidavit was approved. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - COLLEGE FOOTBALL HALL OF FAME - <br />SYNTHETIC SURFACES - PROJECT NO.93-09 <br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manager, has submitted the <br />Project Completion Affidavit on behalf of Kiefer Specialty Flooring, Inc., 1700 Kiefer drive, Zion, <br />Illinois, for the above referred to project, indicating a final cost of $152,917.02. Upon a motion <br />made by Mr. Caldwell, seconded by Mrs. Manier and carried, the Project Completion Affidavit was <br />approved. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - COLLEGE FOOTBALL HALL OF FAME - <br />FLOOR FINISHES - PROJECT NO. 93-09 <br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manger, has submitted the Project <br />Completion Affidavit on behalf Interior Finishes, Inc., Post Office Box 308, Osceola, Indiana, for <br />the above referred to project, indicating a final cost of $223,418.96. Upon a motion made by Mr. <br />Caldwell, seconded by Mrs. Manier and carried, the Project Completion Affidavit was approved. <br />APPROVE GRANT OF LICENSE - A PORTION OF RIGHT-OF-WAY AT LAFAYETTE <br />BOULEVARD - HERRMAN & GOETZ <br />Mr. Leszczynski stated that the Board is in receipt of a Grant of License, as submitted by Herrman <br />& Goetz, 225 South Lafayette Boulevard, South Bend, Indiana, for usage of portion of the right-of- <br />way on Lafayette Boulevard, as submitted to the Board, as a permanent easement to accommodate <br />a fiber optic communication line. Therefore, upon a motion made by Mr. Caldwell, seconded by <br />Mrs. Mariier and carried, the Grant of License was approved and executed. <br />APPROVAL OF REQUEST TO INCREASE CHARGES - COMPOST, WOOD CHIPS. MULCH - <br />ORGANIC RESOURCE FACILITY <br />In a letter to the Board, Mr. Jack Dillon, Director, Environmental Services, requested permission to <br />increase the charge for compost, wood chips and mulch for the Organic Resource Facility. Mr. <br />Dillon recommended that this charge be increased from $2.00 per cubic yard to $4.00 per cubic yard, <br />based on the cost of loading 7,500 cubic yards of material. Additionally, Mr. Dillon recommended <br />that a flat rate tipping fee of $7.00 for pick up trucks with yard waste equal to bed side wall be <br />instituted, and $14.00 for pick ups with side boards or yard waste heaped above the side wall. Mr. <br />Dillon stated that the increased revenues generated would help to balance the program budget, while <br />remaining well below market value.
