REGULAR MEETING <br />APRIL 1. 1996 <br />AWARD BID - ONE (1) 20 YARD REFUSE PACKER <br />Mr. Matthew Chlebowski, Director, Division of Equipment Services, advised the Board that on <br />March 25, 1996, bids were received and opened for the refuse packer. After reviewing those bids, <br />Mr. Chlebowski recommends that the Board award the bid to Best Equipment Company, Inc., 2804 <br />North Catherwood Avenue, Indianapolis, Indiana in the amount of $103,704.00. Mr. Chlebowski <br />stated that this was the lowest bid received, and was the alternate bid, consisting of (1) 1997 WX64 <br />WhiteNolvo with Leach 2R11. Further, funding for this vehicle will come from the Environmental <br />Services Capital Budget. Therefore, Mrs. Manier made a motion that the recommendation be <br />accepted and the bid be awarded as outlined above. Mr. Caldwell seconded the motion which <br />carried. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - COLLEGE FOOTBALL HALL OF FAME - <br />DYE PLUMBING - PROJECT NO. 93-09 <br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manager, has submitted the <br />Project Completion Affidavit on behalf of Dye Plumbing, Post Office Box 1729, LaPorte, Indiana, <br />for the above referred to project, indicating a final cost of $687,901.83. Upon a motion made by Mr. <br />Leszczynski, seconded by Mrs. Manier and carried, the Project Completion Affidavit was approved. <br />APPROVE PROJECT COMPLETION AFFIDAVIT - CREEKWOOD VILLAS SUBDIVISION - <br />PROJECT NO. 89-15 <br />Mr. Leszczynski advised that Mr. Jeff Boss, Division of Engineering, has submitted the Project <br />Completion Affidavit on behalf of Miller Builders, South Bend, Indiana, for the above referred to <br />project. Mr. Leszczynski stated that the Project Completion Affidavit was for the completion of <br />sewer, water, storm drains and roads for the subdivision. Upon a motion made by Mr. Leszczynski, <br />seconded by Mr. Caldwell and carried, the Project Completion Affidavit was approved. <br />APPROVE CHANGE ORDER NO. 1 (FINAL) AND PROJECT COMPLETION AFFIDAVIT - <br />S.E. REDEVELOPMENT AREA SITE CLEARANCE - PHASE II - PROJECT NO. 95-81 <br />Mr. Leszczynski advised that Mr. Bob Allen, Public Construction Manager has submitted Change <br />Order No. 1 (Final) on behalf of Advanced Environmental Systems, 15214 Tonkel Road at Hosler <br />Road, Leo, Indiana, indicating that the contract amount be increased by $5,693.50 for a new contract <br />sum including this Change Order in the amount of $18,009.50. It was noted that Advanced <br />Environmental Systems was awarded the asbestos removal package, while Warner and Sons was <br />awarded the demolition portion. One house, 601 Ohio, was included in the demolition portion but <br />not in the asbestos portion. At the time the bid was let, people were still residing at 601 Ohio. This <br />structure, therefore, could not be inspected to determine the extent of asbestos contamination without <br />endangering the health of the occupants. After the bid was awarded, the house was vacated. At that <br />time it was discovered that all of the siding on the house was asbestos. At the same time, the <br />demolition was put on the "fast track," due to rodent infestation. While it may have been reasonable <br />to assume there was at least some asbestos, it was unforeseeable that the extent of the contamination <br />would be so great, particularly given the minimal levels of asbestos found in surrounding properties. <br />The exact levels of contamination could not be determined without endangering the occupants of the <br />home. There is no change in the intent of this entire bid package, namely, the abatement of asbestos <br />and subsequent demolition of nine homes in the Southeast Redevelopment Area. This change order <br />contemplates only the completion of that original task. The contractor will be paid for the additional <br />work in the same unit prices as shown in the original contract. It was more efficient to proceed with <br />this work with a change order to the contract as opposed to seeking additional bids. Additionally <br />submitted was the Project Completion Affidavit indicating this new final cost. Upon a motion made <br />by Mr. Caldwell, seconded by Mrs. Manier and carried, Change Order No. 1 (Final) and the Project <br />Completion Affidavit were approved. <br />APPROVAL OF AGREEMENT - ENGINEERING SERVICES - SOUTHFIELD LIFT STATION - <br />PROJECT NO. 96-24 <br />Mr. Leszczynski stated that the Board is in receipt of an Agreement between the City of South Bend <br />and The Troyer Group, Inc., 415 Lincolnway East, Mishawaka, Indiana, for professional engineering <br />services for the construction of a sanitary sewer pumping station in Southfield, approximately 900 <br />feet east of Miami Highway and 2,200 feet south of Kern Road, and a force main from the pumping <br />1 <br />1 <br />1 <br />
