100 <br />APPROVE WASTEWATER DISCHARGE PERMIT <br />In a Memorandum to the Board, Mr. Ken Zmudzinski, Pretreatment Coordinator, Division of <br />Environmental Services, submitted the following Wastewater Discharge Permit and recommended <br />approval: <br />1. Swank Uniform Rental <br />1101 King Street <br />South Bend, Indiana <br />Therefore, upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and carried, the <br />Wastewater Discharge Permit was approved. <br />APPROVE AND/OR RELEASE CONTRACTOR AND EXCAVATION BONDS <br />Mr. Larry D. Spradlin, Sr., Engineer, Division of Engineering, recommended that the following <br />Contractors and Excavation Bonds be approved and/or released as follows: <br />BOND OF CONTRACTOR <br />Network Field Services <br />B&B Construction <br />EXCAVATION BONDS <br />Network Field Services <br />Released Effective March 18, 1996 <br />Released Effective March 18, 1996 <br />Released Effective March 18, 1996 <br />Mr. Caldwell made a motion that the recommendation be accepted and that the Bonds be approved <br />and/or released as outlined above. Mr. Leszczynski seconded the motion which carried. <br />FILING OF CONTRACTOR REQUEST FOR EMERGENCY BOARD UP LIST <br />In a letter to the Board, Ms. Julie Perkins, Progressive Builders, Inc., 2512 Lincolnway West, <br />Mishawaka, Indiana, requested to be added to the waiting list of the Emergency Board -up List. Upon <br />a motion made by Mr. Leszczynski, seconded by Mrs. Manier and carried, the request was filed. <br />APPROVE TITLE SHEET - ST. JOSEPH COUNTY GROUNDWATER - MAYFLOWER ROAD <br />SITE - WATER DISTRIBUTION SYSTEM IMPROVEMENTS <br />Mr. Leszczynski advised that the Title Sheet for the above referred to project was being presented <br />at this time for execution. Upon a motion made by Mr. Caldwell, seconded by Mrs. Manier and <br />carried, the above referred to Title Sheet was approved and signed. <br />APPROVE CLAIMS <br />Mr. Kevin Horton, City Controller, submitted claims in the amount of $673,262.68, dated March 11, <br />1996 and $916,223.03, dated March 18, 1996 and recommended approval. Mr. Leszczynski made <br />a motion that the claims be approved and the reports as submitted be filed. Mrs. Manier seconded <br />the motion which carried. <br />i� <br />1 <br />L <br />
