Szuba, Manager Non -Collusion Affidavit was in order Non -Discrimination Commitment form was completed Five per cent (5%) Bid Bond was submitted BID: $168,112.33 LAPORTE CONSTRUCTION CO. INC. Post Office Box 577 LaPorte, Indiana 46352 Bid was signed by Mr. Thomas Blackburn, President Non -Collusion Affidavit was in order Non -Discrimination Commitment form was completed Five per cent (5%) Bid Bond was submitted BID: $199,336.40 Upon a motion made by Mr. Caldwell, seconded by Ms. Littrell and carried, the above bids were referred to the Division of Engineering for review and recommendation. AWARD OF BIDS FOR OLIVE ROAD COMMERCE PARK TABLED - PROJECT NO. 96-17 Upon a motion made by Mr. Caldwell, seconded by Mr. Littrell and carried, the bid award for the above referred to project was tabled. APPROVE CHANGE ORDERS - CLEVELAND ROAD AND BENDIX DRIVE INTERSECTION IMPROVEMENTS - PROJECT NO 97-10 Mr. Caldwell advised that Mr. Toy Villa, Public Construction Manager, has submitted the following Change Orders on behalf of Rieth Riley Construction Company, Post Office Box 1775, South Bend, Indiana and recommended approval: Change Order Amount Contract Sum 1 Increase $12,915.00 $796,347.32 2 Increase $ 4,403.07 $800,750.39 Upon a motion made by Mr. Caldwell, seconded by Mr. Littrell and carried, the Change Orders were approved. APPROVE CHANGE ORDER - OLIVE/LOCUST ROAD SEWER STREET IMPROVEMENTS - PROJECT NO. 96-60 Mr. Caldwell advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order No. 2 on behalf of Walsh and Kelly, Inc., 24358 State Road 23, South Bend, Indiana indicating that the completion date be extended due to the additions of Alternate Phases 1 and 2 which did not include working days. This Change Order was not a monetary increase or decrease. Upon a motion made by Mr. Caldwell, seconded by Mr. Littrell and carried, the Change Order was approved. APPROVE CHANGE ORDER - IRONWOOD WIDENING - R-23194 - PROJECT NO. 96-25 Mr. Caldwell advised that Mr. Darrell Baker, Project Engineer, has submitted Change Order No. 1, on behalf of Small Inc., 25190 State Road 4, North Liberty, Indiana, indicating that the Contract amount be increased $3,872.00 for a new Contract sum including this Change Order in the amount of $1,066,148.17. Upon a motion made by Mr. Caldwell, seconded by Ms. Roemer and carried, the Change Order was approved.
