<br />APPROVE CHANGE ORDER - MORRIS PERFORMING ARTS CENTER - LEAD BASER
<br />PAINT ABATEMENT - PROJECT NO. 98-25-5.2
<br />Mr. Caldwell advised that Mr. Toy Villa, Construction Manager, has submitted Change Order No.
<br />001 on behalf of Clean Air Systems, Inc., 23349 State Road 23 West, South Bend, Indiana indicating
<br />that the Contract amount be increased $2,300.00 for a new Contract sum including this Change Order
<br />in the amount of $41,890.00. Upon a motion made by Mr. Powell, seconded by Ms. Roemer and
<br />carried„the Change Order was approved.
<br />APPROVE CHANGE ORDER - MORRIS PERFORMING ARTS CENTER - LEAD BASED
<br />PAINT ABATEMENT - PROJECT NO.98-25-5.2
<br />Mr. Caldwell advised that Mr. Toy Villa, Construction Manger, has submitted Change OrderNo. 002
<br />on behalf of Clean Air Systems, Inc., 23349 State Road 23 West, South Bend, Indiana indicating that
<br />the Contract amount be increased $12,625.00 for a new Contract sum including this Change Order
<br />in the amount of $54,515.00. Upon a motion made by Ms. Roemer, seconded by Mr. Powell and
<br />carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER _-_MORRIS PERFORMING ARTS CENTER - EXCAVATION
<br />AND SHEETING - PROJECT NO. 98-25-3A
<br />Mr. Caldwell advised that Mr. Toy Villa, Construction Manager, has submitted Change Order No.
<br />003 (revised) on behalf of Ritschard Brothers, Inc., 1204 West Sample, South Bend, Indiana
<br />indicating that the Contract amount be increased $3,630.00 for a new Contract sum including this
<br />Change Order in the amount of $252,960.00. Upon a motion made by Mr. Powell, seconded by Ms.
<br />Roemer and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER - MORRIS PERFORMING ARTS CENTER - MASONRY AND
<br />MASONRY RESTORATION - PROJECT NO. 98-25-5.4
<br />Mr. Caldwell advised that Mr. Toy Villa, Construction Manager, has submitted Change Order No.
<br />001 on behalf of Ramenda Masonry Company, Inc., 54889 Lexington Avenue, South Bend, Indiana.
<br />This Change Order removes and installs the masonry, of which the amount balances out. There is no
<br />monetary increase/decrease for this Change Order. The Contract Sum remains at $551,600.00.
<br />Upon a motion made by Mr. Caldwell, seconded by Ms. Roemer and carried, the Change Order was
<br />approved.
<br />APPROVE CHANGE ORDER - MORRIS PERFORMING ARTS CENTER - CONCRETE -
<br />PROJECT NO. 98-25-313
<br />Mr. Caldwell advised that Mr. Toy Villa, Construction Manager, has submitted Change Order No.
<br />004 (revised) on behalf of The Robert Henry Corporation, Post Office Box 1407, South Bend,
<br />Indiana indicating that the Contract amount be increased $24,389.00 for a new Contract sum
<br />including this Change Order in the amount of $678,904.00. Upon a motion made by Ms. Roemer,
<br />seconded by Mr. Powell and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER - NORTH PUMPING STATION IMPROVEMENTS - PROJECT
<br />NO. 98-92
<br />Mr. Caldwell advised that Mr. Toy Villa, Construction Manager, has submitted Change Order No.
<br />4 onbehalfofNorth America Construction, 3500 West Highway 13, Burnsville, Minnesota indicating
<br />that the Contract amount be increased $58,531.00 for a new Contract sum including this Change
<br />Order in the amount of $6,154,674.00. Upon a motion made by Mr. Caldwell, seconded by Ms.
<br />Roemer and carried, the Change Order was approved.
<br />APPROVE AGREEMENT BETWEEN OWNER AND CONTRACTOR - LEIGHTON PLAZA -
<br />PHASE III - LEASE IMPROVEMENTS FOR LOTS 7 AND 8 - PROJECT NO. 97-57-10
<br />Mr. Caldwell advised that in accordance with the bid awarded on July 19, 1999, to Forsey
<br />Construction, 2018 Ironwood Circle, South Bend, Indiana, in the amount of $40,700.00 for the above
<br />referred to project, a Contract in said amount was being submitted for Board approval. Upon a
<br />motion made by Ms. Roemer, seconded by Mr. Caldwell and carried, the Contract was approved and
<br />the appropriate Certificate of Insurance as submitted was filed.
