This is EXHIBIT A, consisting of 14 pages, referred to in <br />and part of the Agreement between Owner and Engineer <br />for Professional Services dated <br />Engineer's Services <br />Article 1 of the Agreement is supplemented to include the following agreement of the parties. <br />Engineer shall provide Basic and Additional Services, for the new sidewalk along Corby Boulevard <br />between Twyckeham Drive and Churchill Drive. It is undersood that the City of South Bend has a limited <br />budget for the first phase of sidewalk installation, which is to be installed in 2016. Therefore, the final <br />Construction Documents and Specification shall identify segments of the sidewalk as Alternates for <br />phasing purposes. <br />The services for this project are as set forth below <br />PART 1— BASIC SERVICES <br />A1.01 Topographic Survey <br />A. The Engineer has reviewed the proposed concept drawing, aerial photographs as well as driven the <br />site limits, shown in Attachment B, to obtain a thorough understanding of the task at hand. The <br />project study area is located on Corby Boulevard and is between Twyckeham Drive and east of <br />Churchill Drive. There is approximately 2,100 lineal feet of roadway within the project limits. <br />B. The Engineer will provide the following information as it pertains to the survey tasks: <br />1. Establish both horizontal and vertical control throughout the project limits utilizing both <br />GPS survey grade equipment as well as conventional survey equipment. Vertical control <br />datum is to be on the NAVD88 system. <br />2. Request utility locates per the Indiana 811 Call Before You Dig call center. Please note <br />that the utilities that will be marked by this company do not include any private utilities. <br />Should private utilities exist within the project limits it may be necessary to contact a <br />private utility locating company to have these lines marked in the field. Fees associated <br />with a private utility locating company are provided as an Allowance, as identified in <br />Exhibit C. <br />3. Correspondence with utility companies to facilitate the locate process. This may include <br />requesting record utility information so the location of the utilities may be graphically <br />depicted on the drawings. <br />4. Collect the topographic information within the project limits, including but not limited to, <br />sanitary and storm structures, edges of pavement and curb lines, building comers, observed <br />utilities, trees and spot elevations. <br />5. Collect invert information for the sanitary and storm structures within the project limits. <br />6. Perform research to acquire subdivision plats, prior surveys, and roadway plans. <br />7. Prepare a CAD drawing of the existing site conditions that will include a surface model <br />with contours for design purposes. <br />8.
