REGULAR MEETING OCTOBER 8, 2001 by Mr. Gilot, seconded by Mr. Inks and carried, the report was accepted and filed. FILING OF CERTIFICATES OF INSURANCE Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the following Certificates of Insurance were accepted for filing: First Source Corporation Post Office Box 1602 South Bend, Indiana 46634-1602 Small, Inc. 25190 State Road 4 North Liberty, Indiana 46554 Pierce Manufacturing, Inc. Post Office Box 2017 Appleton, Wisconsin 54913-2017 Centex Rodgers, Inc. 2636 Elm Hill Pike Nashville, Tennessee 37214 Majority Builders, Inc. 62900 U.S. 31 South South Bend, Indiana 46614 Lawson Fisher Associates 525 West Washington South Bend, Indiana 46601 Papczynski Construction, Inc. 66717 Millet Road Lakeville, Indiana 46536 YWCA of St. Joseph County, Inc. 802 North Lafayette South Bend, Indiana 46601 APPROVE TITLE SHEETS - MIAMI AND RIDGEDALE - PROJECT NO. 100-90 - MAYFLOWER - SECTION 10 - EASEMENT SANITARY SEWER Mr. Gilot advised that the Title Sheets for the above referred to projects were being presented at this time for execution. Upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, the above referred to Title Sheets were approved and signed. APPROVE EXCAVATION BONDS Mr. Anthony Molnar, Engineer, Division of Engineering, recommended that the following Excavation Bonds be approved as follows: EXCAVATION BONDS Greg Johnson Excavating, Inc. Approved Effective September 27, 2001 Pursuant to Res. No. 100-2000 Sky Plumbing LLC Approved Effective September 27, 2001 Pursuant to Res. No. 100-2000 Mr. Inks made a motion that the recommendation be accepted and that the Bonds be approved as outlined above. Mr. Littrell seconded the motion which carried. APPROVE CONSENT TO ANNEXATION AND WAIVER OF RIGHTS TO REMONSTRATE Mr. Gilot stated that the Board is in receipt of a Consent to Annexation and Waiver of Right to Remonstrate as submitted by Mr. Michael Edmonds, 26405 Edison Road, South Bend, Indiana. The Consent indicates that in consideration for permission to tap into the public sanitary sewer system of the City, to provide sanitary sewer service to the east side of Quince Road on the north side of Edison Road, South Bend, Indiana, (Key # 21-1043-0729), Mr. Edmonds waives and releases any and all right to remonstrate against or oppose any pending or future annexation of the property by the City of South Bend. Therefore, Mr. Littrell made a motion that the Consent be approved. Mr. Inks seconded the motion which carried. APPROVE CONSENT TO ANNEXATION AND WAIVER OF RIGHTS TO REMONSTRATE Mr. Gilot stated that the Board is in receipt of a Consent to Annexation and Waiver of Right to Remonstrate as submitted by Ms. Connie Moon, 53265 Nadine Street, South Bend, Indiana. The Consent indicates that in consideration for permission to tap into the public water system of the City, to provide sanitary water service to the south side of Cleveland Road on the west side of Nadine Street, South Bend, Indiana, (Key # 02-1020-055215), Ms. Moon waives and releases any and all 1 1 1
