REGULAR MEETING JUNE 24, 2002 APPROVE MINUTES OF PREVIOUS MEETING Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the minutes of the regular meeting of the Board held on June 10, 2002 and a special meeting held on June 17, 2002 were approved. OPENING OF QUOTATIONS - MIAMI VILLAGE PARK - BRICKWORK AND STONEWORK - PROJECT NO. 102-025 This was the date set for receiving and opening of sealed quotations for the above referred to project. The following quotations were opened and publicly read: KASER SPRAKER CONSTRUCTION 25487 West State Road 2 South Bend, Indiana 46619 QUOTATION: $36,765.00 RAMENDA MASONRY 54889 Lexington Avenue South Bend, Indiana 46628 QUOTATION: $68,280.00 ZIOLKOWSKI CONSTRUCTION INC. 1005 South Lafayette South Bend, Indiana 46601 QUOTATION: $38,700.00 Upon a motion made by Mr. Littrell, seconded by Mr. Inks and carried, the above quotations were referred to the Division of Engineering for review and recommendation. AWARD BID - OLIVER PLOW WORKS - SITE DEMOLITION - PHASE I - PROJECT NO. 100- 092A Mr. Carl P. Littrell, City Engineer, advised the Board that on June 17, 2002, bids were received and opened for the above referred to project. After reviewing those bids, Mr. Littrell recommends that the Board award the contract to Bianchi Industrial Services LLC, 208 Long Branch Road, Syracuse, New York, in the amount of $263,581.00. Therefore, Mr. Inks made a motion that the recommendation be accepted and the bid be awarded as outlined above. Mr. Littrell seconded the motion which carried. APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - LEEPER PARK/RIVERSIDE DRIVE PARKING IMPROVEMENTS - RIETH RILEY CONSTRUCTION - PROJECT NO. 101-035 Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final Change Order on behalf of Rieth Riley Construction, Post Office Box 1775, South Bend, Indiana, indicating that the contract amount be decreased by $2,882.28 for a new contract sum including this Change Order in the amount of $96,651.68. Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the Final Change Order and the Project Completion Affidavit were approved subject to the filing of the appropriate three-year Maintenance Bond. APPROVE FINAL CHANGE ORDER AND PROJECT COMPLETION AFFIDAVIT - KELLY PARK IMPROVEMENTS - RIETH RILEY CONSTRUCTION - PROJECT NO. 101-022 Mr. Gilot advised that Mr. Toy Villa, Public Construction Manager, has submitted the Final Change Order on behalf of Rieth Riley Construction, Post Office Box 1775, South Bend, Indiana, indicating that the contract amount be decreased by $197.90 for a new contract sum including this Change Order in the amount of $27,290.65. Additionally submitted was the Project Completion Affidavit indicating this new final cost. Upon a motion made by Mr. Inks, seconded by Mr. Littrell and carried, the Final Change Order and the Project Completion Affidavit were approved subject to the filing of the appropriate three-year Maintenance Bond.
