<br />including this Change Order in the amount of $11,804,057.00. Upon a motion made by Mr. Littrell,
<br />seconded by Mr. Inks and carried, the Change Order was approved.
<br />APPROVE CHANGE ORDER - TJX SITE - GRADING AND DRAINAGE PROJECT - RIETH
<br />RILEY CONSTRUCTION - PROJECT NO. 102-034A
<br />Mr. Littrell advised that Mr. Toy Villa, Public Construction Manager, has submitted Change Order
<br />No. 1 on behalf of Rieth Riley Construction, Post Office Box 1775, South Bend, Indiana indicating
<br />that the Contract amount be decreased $22,688.00 for a new Contract sum including this Change
<br />Order in the amount of $1,762,822.00. Upon a motion made by Mr. Littrell, seconded by Mr. Inks
<br />and carried, the Change Order was approved.
<br />APPROVE STANDARD FORM OF AGREEMENT BETWEEN OWNER AND CONTRACTOR -
<br />SOUTH BEND WASTEWATER TREATMENT PLANT - FILTER PRESS RENOVATION -
<br />ASHBROOK CORPORATION - PROJECT NO. 102-013
<br />Mr. Littrell advised that in accordance with the bid awarded to Ashbrook Corporation, 11600 East
<br />Hardy, Houston, Texas, in the amount of $371,360.00 for the above referred to project, a Contract
<br />in said amount was being submitted for Board approval. Upon a motion made by Mr. Littrell,
<br />seconded by Mr. Inks and carried, the Contract was approved and the appropriate Certificate of
<br />Insurance, Performance Bond and Labor and Materials Payment Bond as submitted were filed.
<br />APPROVAL OF AGREEMENT - WORKFORCE DIVERSITY INITIATIVE - WORKFORCE
<br />DEVELOPMENT
<br />Mr. Littrell stated that the Board is in receipt of an Agreement between the City of South Bend and
<br />Workforce Investment Board, 401 East Colfax, South Bend, Indiana, for continued support of
<br />Workforce Diversity program for business outreach and job placement activities of WDS. Mr.
<br />Littrell noted that the Agreement is in the amount of $100,000.00. Therefore, upon a motion made
<br />by Mr. Inks, seconded by Mr. Littrell and carried, the Agreement was approved and executed.
<br />APPROVAL OF PROPOSAL - GEOTECHNICAL EVALUATION - RECONSTRUCTION OF
<br />NIMTZ PARKWAY AND OLIVE ROAD - SHILTS, GRAVES & ASSOCIATES
<br />Mr. Littrell stated that the Board is in receipt of a Proposal between the City of South Bend and
<br />Shiltz, Graves & Associates, 4310-C Technology Drive, South Bend, Indiana, for geotechnical
<br />evaluation. Mr. Littrell noted that the Proposal is in the amount of $4,587.50. Therefore, upon a
<br />motion made by Mr. Littrell, seconded by Mr. Inks and carried, the Proposal was approved and
<br />executed.
<br />APPROVAL OF PROPOSAL - EXTENDED PHASE II ENVIRONMENTAL SITE ASSESSMENT
<br />AT WHITE LIGHTING FACILITY (520, 522 LINCOLNWAY WEST AND 335 KUNTSMAN
<br />COURT) - HULL AND ASSOCIATES
<br />Mr. Littrell stated that the Board is in receipt of a Proposal between the City of South Bend and Hull
<br />& Associates, 4700 Duke Drive, Mason, Ohio, for the above referred to project. Mr. Littrell noted
<br />that the Proposal is in the amount of $26,028.00, funded by a Grant. Therefore, upon a motion made
<br />by Mr. Littrell, seconded by Mr. Inks and carried, the Proposal was approved and executed, subject
<br />to finalization of financing.
