0290 <br />REGULAR MEETING AUGUST 11, 2003 <br />Mr. Gilot noted that the Agreement is in the amount of $22,900.00, bringing the contract amount to <br />$356,367.40. Therefore, upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, the <br />Supplemental Agreement was approved and executed. <br />RESCIND PREVIOUS CONTRACT/APPROVAL OF REVISED CONTRACT - UTILIZATION <br />OF USTFIELDS PILOT GRANT FUNDING - INDIANA DEPARTMENT OF <br />ENVIRONMENTAL MANAGEMENT <br />Mr. Gilot stated that the Board is in receipt of revised Contract between the City of South Bend and <br />the Indiana Department of Environmental Management (IDEM), for environmental assessments of <br />properties with underground storage tanks along South Bend's commercial corridors. Mr. Gilot <br />noted that the Contract is in the amount of a 1.0% match to a grant amount coming from IDEM. It <br />was noted that this Contract replaces a prior Contract (ARN#305-3-722) which the Board of Public <br />Works previously approved. The new Contract extends the program time. Therefore, upon a motion <br />made by Mr. Littrell, seconded by Mr. Inks and carried, the Contract was approved and executed. <br />APPROVAL OF AGREEMENT - PLANNING SERVICES FOR DEVELOPMENT OF CITY OF <br />SOUTH BEND ZONING ORDINANCE - ST. JOSEPH COUNTY BOARD OF <br />COMMISSIONERS <br />Mr. Gilot stated that the Board is in receipt of an Agreement between the City of South Bend and <br />the St. Joseph County Board of Commissioners, for the above referred to project. Mr. Gilot noted <br />that the Agreement is in the amount of $25,000.00. Therefore, upon a motion made by Mr. Gilot, <br />seconded by Mr. Littrell and carried, the Agreement was approved and executed. <br />APPROVE STANDARD FORM OF AGREEMENT BETWEEN OWNER AND CONTRACTOR - <br />ANAEROBIC SLUDGE DIGESTER CLEANING - WASTEWATER TREATMENT PLANT - <br />SYNAGRO MIDWEST, INC. - PROJECT NO. 103-035 <br />Mr. Gilot advised that in accordance with the bid awarded on July 14, 2003, to Synagro Midwest, <br />Inc.,1014 East Baltimore Street, Baltimore, Maryland, in the amount of 1112,900.00 for the above <br />referred to project, a Contract in said amount was being submitted for Board approval. Upon a <br />motion made by Mr. Gilot, seconded by Mr. Littrell and carried, the Contract was approved and the <br />appropriate Certificate of Insurance, Performance Bond and Labor and Materials Payment Bond as <br />submitted were filed. <br />APPROVAL OF AMENDMENT TO CONTRACT - CHANGE OF DESIGN SCOPE - WALNUT <br />AND EUCLID CONTROL STRUCTURE - LAWSON FISHER ASSOCIATES - PROJECT NO. <br />102-078 <br />Mr. Gilot stated that the Board is in receipt of an Amendment to a Contract between the City of <br />South Bend and Lawson Fisher Associates, 525 West Washington, South Bend, Indiana, for the <br />above referred to project. Mr. Gilot noted that the Amendment is in the amount of $4,650.00, <br />bringing the contract amount to $28,050.00. Therefore, upon a motion made by Mr. Gilot, seconded <br />by Mr. Littrell and carried, the Amendment to the Contract was approved and executed. <br />APPROVAL OF PURCHASE AGREEMENT - SALE OF CITY -OWNED PROPERTY - 816 <br />SOUTH JACKSON <br />Mr. Gilot stated that the Board is in receipt of an Agreement between the City of South Bend and <br />Mr. Claude Anderson, 812 South Jackson, South Bend, Indiana, for the sale of the above referred <br />to property. Therefore, upon a motion made by Mr. Inks, seconded by Mr. Littrell and carried, the <br />Agreement was approved and executed. <br />APPROVAL OF ADDENDUM No. 2 - RIVERSIDE DRIVE MULTIPLE USE PATH - THE <br />TROYER GROUP <br />Mr. Gilot stated that the Board is in receipt of an Addendum to an Agreement between the City of <br />South Bend and The Troyer Group, 550 Union Street, Mishawaka, Indiana, for the completion of <br />plans and specifications for a section of the Riverside Drive Multiple Use Path. Mr. Gilot noted that <br />the Addendum is in the amount of $65,450.00. Therefore, upon a motion made by Mr. Inks, <br />seconded by Mr. Littrell and carried, the Addendum was approved and executed. <br />
