REGULAR MEETING DECEMBER 12, 2005 <br />WHEREAS, the Indiana State Board of Accounts has instructed the City to write <br />off all uncollectible accounts after every attempt to collect them has been made; and <br />WHEREAS, the City of South Bend, by and through its Board of Public Works. <br />desires to write off the accounts listed on the attached documents. <br />NOW, THEREFORE, BE IT RESOLVED by the Board of Public Works of the <br />City of South Bend, that the attached documents totaling $1,390.60 be written off to the <br />accounts receivable of the City of South Bend as uncollectible. <br />ADOPTED this 121h Day of December 2005. <br />BOARD OF PUBLIC WORKS <br />s/Gary A. Jacob, Clerk <br />FILING OF SAFETY REPORTS - DEPARTMENT OF PUBLIC WORKS <br />The Division of Environmental Services, Street Department, Division of Engineering, <br />Water Works and Equipment Services submitted Safety Reports for November 2005. <br />These reports will reflect injuries/accidents for each month and provide for a comparison. <br />There being no further discussion, upon a motion made by Mr. Gilot, seconded by Mr. <br />Littrell and carried, the reports were accepted and filed. <br />FILING OF MONTHLY REPORT - ENVIRONMENTAL SERVICES <br />Mr. Jack Dillon, Director of Environmental Services, submitted a report for the Division <br />of Environmental Services, for the Wastewater Treatment Plant, Organic Resources. <br />Solid Waste and Sewer Repair, for the month of November 2005. There being no further <br />discussion, upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried. tlhc <br />monthly reports were accepted and filed. <br />FILING OF MONTHLY REPORT - WATER WORKS <br />Mr. Andy Wierzbicki, Water Works, submitted a report for the Division of Water Works <br />for the month of November 2005. There being no further discussion, upon a motion <br />made by Mr. Gilot, seconded by Mr. Littrell and carried, the monthly report was accepted <br />and filed. <br />APPROVE WASTEWATER DISCHARGE PERMIT <br />In a Memorandum to the Board, Mr. Manuel Bueno, Pretreatment Coordinator, Division <br />of Environmental Services, submitted the following Wastewater Discharge Permit and <br />recommended approval: <br />Ark Engineering Services, Inc. For Remediation works at: <br />I&M Deli Service Station <br />702 Lincolnway East <br />South Bend, Indiana <br />Upon a motion made by Mr. Littrell, seconded by Mr. Inks and carried, the Wastewater <br />Discharge Permit was approved. <br />APPROVE TRAFFIC CONTROL DEVICE <br />Upon a motion made by Mr. Gilot, seconded by Mr. Littrell and carried, the following <br />traffic control device was approved: <br />NEW INSTALLATION: No Parking 7 a.m. to 9 a.m. <br />LOCATION: Colfax, Main Street East to Michigan, South <br />Side <br />
