±JFM 0120 TYPE BUSINESS DESCRIPTION AMOUNT Supplemental Revision — Michigan Street Lawson Fisher Associates $229,840.00 Agreement Improvements — Ireland to No.1 Chippewa Proposal Neighborhood Traffic Abonmarche Consultants $28,900.00 Calming Program Planning Supplemental Ewing Avenue Ken Herceg and $96,900.00 Agreements Reconstruction — Prairie Associates Nos. 1 and 2 1 Avenue to Michigan Street APPROVE WASTEWATER DISCHARGE PERMIT In a Memorandum to the Board, Mr. Manuel Bueno, Pretreatment Coordinator, Division of Environmental Services, submitted the following Wastewater Discharge Permit and recommended approval: Federal Mogul 3605 West Cleveland Road South Bend, Indiana Upon a motion made by Mr. Gilot, seconded by Mr. Inks and carried, the Permit was approved. ADOPT RESOLUTION NO. 24-2005 — ADOPTING AN AMENDED WRITTEN FISCAL PLAN AND ESTABLISHING A POLICY FOR THE PROVISION OF SERVICES TO AN ANNEXATION AREA IN GERMAN TOWNSHIP (WAGGONER'S DAIRY FARM INC. ANNEXATION AREA) Upon a motion made by Mr. Inks, seconded by Mr. Littrell and carried, the following Resolution was adopted by the Board of Public Works: WHEREAS, on March 28, 2005, by Resolution No. 22-2005, this Board of Public Works adopted a Written Fiscal Plan for the proposed annexation area; and WHEREAS, the Written Fiscal Plan which this Board approved on March 28, 2005 has been amended to include additional information relevant to the provision of fire protection services to the annexation territory, but has not been amended in any other way; and WHEREAS, the amended terms of the Fiscal Plan to not affect this Board of Public Work's prior approval of the Fiscal Plan as it applies to services over which this Board exercises control, i.e., services of a no capital improvement nature such as street and road maintenance, street sweeping, flushing, show removal and sewage collection, as well as services of a capital improvement nature including street and road construction, street lighting, a sanitary sewer system, a water distribution system, a storm water system and drainage plan; and WHEREAS, the Board of Public Works now wishes to reaffirm and incorporate its approval of the previously submitted Fiscal Plan as well as the terms of the amended Fiscal Plan attached hereto and incorporated herein as Exhibit "A". NOW, THEREFORE, E IT RESOLED BY THE BOARD OF PUBLIC WORKS OF THE CITY OF SOUTH BEND, AS FOLLOWS: Section I. The terms of the Board of Public Works Resolution No. 22-2005 dated Mach 28, 2005 adopting a Fiscal Plan for the furnishing of services to the territory to be annexed is are affirmed. Section II. The terms of the Amended Fiscal Plan which is attached hereto as Exhibit "A" and made a part hereof for the furnishing of services to this annexation area which include new terms for the expansion of fire protection services to the annexation area is also approved and adopted by this Board of Public Works.
