<br />APPROVE EXTENSION OF BID AWARD — TWO (2)PRE-OWNED MID -SIZE
<br />AUTOMOBILES — ENGINEERING AND FIRE DEPARTMENT (LEASE
<br />PURCHASE)
<br />Mr. Lou Grounds, Central Services, advised the board that on June 14, 2004, bids were
<br />received and opened for the above referred to equipment. The request is an extension of
<br />the bid award, in the amount of $21,600.00, to Pine Chevrolet, 1522 East Lincolnway,
<br />LaPorte, Indiana. Therefore, Mr. Gilot made a motion that the recommendation be
<br />accepted and the extension of bid award be approved as outlined above. Mr. Littrell
<br />seconded the motion, which carried.

<br />AUTOMOBILES — CODE ENFORCEMENT (LEASE PURCHASE
<br />Mr. Lou Grounds, Central Services, advised the board that on April 26, 2004, bids were
<br />received and opened for the above referred to equipment to Pine Chevrolet, 1522 East
<br />Lincolnway, LaPorte, Indiana. The request is an extension of the bid award, in the
<br />amount of $18,400.00. Therefore, Mr. Gilot made a motion that the recommendation be
<br />accepted and the extension of bid award be approved as outlined above. Mr. Littrell
<br />seconded the motion, which carried.
<br />AWARD BID — 2005 GOOD NEIGHBORS PROGRAM — PROJECT NO. 105-002
<br />ZONES 1 AND 2 (LRSA)
<br />Mr. Toy Villa, Engineering, advised the board that on March 24, 2005, bids were
<br />received and opened for the above referred to project. After reviewing those bids, Mr.
<br />Villa recommends that the Board award the contract to L.L. Geans Construction, 1923
<br />North Home, Mishawaka, Indiana, the amount of $259,800.00 for Zone 1 and
<br />$239466.00 for Zone 2. Therefore, Mr. Littrell made a motion that the recommendation
<br />be accepted and the bid be awarded as outlined above. Mr. Gilot seconded the motion,
<br />which carried.
<br />APPROVAL OF REQUEST TO ADVERTISE FOR THE RECEIPT OF BIDS — 2005
<br />CONCRETE PAVEMENT REPAIRS — PROJECT NO. 105-033 (LRSA)
<br />In a memorandum to the Board, Mr. Toy Villa, Engineering, requested permission to
<br />advertise for the receipt of bids for the above referred to project. Therefore, upon a
<br />motion made by Mr. Gilot, seconded by Mr. Littrell, the above request was approved.

<br />In a memorandum to the Board, Mr. Lou Grounds, Central Services, requested
<br />permission to advertise for the receipt of bids for the above referred to equipment.
<br />Therefore, upon a motion made by Mr. Gilot, seconded by Mr. Littrell, the above request
<br />was approved.
<br />APPROVE CHANGE ORDER — BENDIX DRIVE RELOCATION — PROJECT NO
<br />102-037 (TIF)
<br />Mr. Gilot advised that Mr. Toy Villa, Engineering, has submitted Change Order No. 4 on
<br />behalf of Brooks Construction Company, Inc., 625 South Beiger, Mishawaka, Indiana,
<br />indicating that the Contract amount be increased $33,790.00, for a new Contract sum
<br />including this Change Order in the amount of $1,356,148.21. Upon a motion made by
<br />Mr. Gilot, seconded by Mr. Littrell and carried, the Change order was approved.
<br />APPROVAL OF AGREEMENT/ADDENDUM
<br />Upon a motion made by Mr. Littrell, seconded by Mr. Gilot and carried, the following
<br />Agreement and Addendum were approved by the Board of Public Works:
