FY 2008 WEED AND SEED COMMUNITIES APPLICATION <br />{ <br />Performance Measures: (1) # of people participating in training and facilitation <br />workshops. <br />Outcomes: (1) A greater number of residents and business owners who understand the <br />differences in thinking between people in poverty and people not in poverty. (2) A greater <br />number of people with sustained ability to pull themselves out of poverty. <br />Seed Program: Community Access Network or "can" and the Family Development <br />Program is a United Way of St. Joseph County initiative to link local nonprofit agencies, <br />government, schools, health, and faith-based organizations together in an electronic-web <br />based network. The HMIS system is a HIPPA compliant, secure database that keeps client <br />and agency information secure. This network will create a ready access to services, share <br />information with multiple agencies at once, and simplify the process. Using the Family <br />Development Case Management system from the University of Iowa, local providers will <br />assist clients in assessing their status, setting goals, and making measurable progress to <br />achieve self-sufficiency. <br />Implementation Status: The first-round of agencies has been recruited and are being <br />trained this fall on both the software and the case management side of the program; they <br />expect to be up and running by December, 2007. Initial funding, from local sources and <br />Liliey Foundation, totaling $386,000, is in place. <br />Performance Measures: (1) # number of organizations in network. (2) # of case <br />managers trained in family development and outcome measurements. (3) # number of <br />clients participating in the network. (4) # number of clients working with case managers <br />on self-sufficiency goals. (5) # clients that achieve self-sufficiency goals. <br />Outcomes: (1) Local agencies and organizations working collaboratively for more <br />efficient delivery of services. (2) Clients achieving measurable changes toward self <br />sufficiency. (3) Through the "can" reporting process, identify areas of need and <br />opportunities to make investments to improve lives. (4) Establishment of a common base <br />of training and support tools for local providers that will result in better client service <br />delivery. (S) The establishment of a community network with which new initiatives can be <br />more readily launched. <br />Seed Program: South Bend Dollar Home Program. The City is working with three <br />neighborhood development corporations to ready, market, and oversee the sale and rehab <br />of homes, in target areas to include the W&S area, for single family homeownership. <br />Implementation Status: Houses are being identi>ied and purchased. <br />Performance Measures: (1)# of homes rehabbed and sold <br />Outcomes: (1) Rehab and sale of homes will increase homeownership and assist in <br />stabilizing the area. <br />V. PROPOSED DEVELOPMENT PLANS <br />V. A. MULTI-YEAR PLAN <br />Plan implementation will begin in Year 1. Year 1 programs will be continued into Year 2 when two <br />new programs will begin implementation. Evaluation of all programs will be ongoing as the <br />Steering Committee will conduct continual review through sub-committee and Steering <br />Committee meetings. Partnerships with Indiana University South Bend and the University of <br />Notre Dame will provide assistance and support to review evaluation methodologies.
