14 <br />EXHIBIT B <br />Project Plan <br />The Developer will complete the following work in accordance with the terms and <br />conditions of this Agreement and in compliance with all applicable laws and regulations: <br />●Developer will redevelop the Property into a no less than one hundred and ten (110) <br />bed Lower-Barrier Shelter. <br />●The Provider shall ensure that the shelter is fully operational and open to receive guests no <br />later than thirty-six (36) months after Construction Commencement Date defined in the <br />Purchase Agreement, or another date agreed to in writing by Parties, subject to all <br />applicable permitting and regulatory approvals. <br />The Developer will complete the work contemplated herein in accordance with the terms and <br />conditions of this Agreement and in compliance with all applicable laws and regulations. The <br />structures will be considered complete upon the issuance of Certificates of Occupancy. <br />The Developer shall also enter into a five-year Operating Agreement with the City of South <br />Bend describing the required operations of the homeless intake center upon completion, which <br />shall include, at a minimum, the requirement for the Developer to follow each of the following <br />terms: <br />1.Operate low-barrier emergency shelter beds year-round (365 days), ensuring <br />accessibility to individuals regardless of sobriety, income, identification, or <br />participation in services. <br />2.Deliver comprehensive emergency shelter services that are Housing-Focused, <br />Person-Centered, Trauma-Informed, and low-barrier. NDIC will implement <br />practices that promote equity, dignity, and rapid housing outcomes, while <br />ensuring guest safety, engagement, and connection to community resources. <br />3. Work collaboratively with OrgCode Consulting to assist in the development and <br />implementation of Standard Operating Procedures (SOPs) necessary for the <br />effective and compliant operation of the shelter. Such SOPs shall be completed <br />and approved prior to the commencement of shelter operations. <br />4. Provide immediate and equitable access to basic needs, including three daily <br />meals, hygiene supplies, showers, laundry services, storage, mailing address, and <br />access to phones, internet, and computers. <br />5. Conduct Diversion conversations with all new guests at intake to explore safe <br />alternatives to shelter, document outcomes, and reduce unnecessary shelter stays. <br />6. Ensure that 100% of guests meet with a Housing-Focused Case Manager within <br />48 hours of entry to begin developing a tailored Housing Plan within 7 days. <br />7. Conduct a triage of new shelter guests within 14 days of shelter entry using <br />approved Coordinated Entry tools to determine acuity and support prioritization <br />for housing resources. <br />8. Develop individualized Housing Plans focused on rapid exit to Permanent <br />Housing, including identification of barriers, action steps, and coordination with
